目的:探讨妊娠期高血压疾病患者胎儿脐动脉S/D比值变化与围产儿结局的影响。方法:随机选择妊娠期高血压疾病患者63例及无任何内外科合并症的初产妇60例,对正常妊娠和妊娠期高血压疾病孕妇进行胎儿脐动脉血流检测,比较两者胎儿脐动脉S/D比值以及与围生儿预后的相关关系。结果:妊娠期高血压疾病组孕龄、胎儿体重低于正常组,收缩压、舒张压S/D比值高于正常组,差异均有统计学意义(P<0.01);妊娠期高血压疾病组胎儿脐动脉S/D比值高于正常妊娠组,差异有统计学意义(P<0.01)。且胎儿脐动脉S/D比值与胎儿体重呈负相关(P<0.01)。结论:先兆子痫子宫胎盘血流灌注明显减少,异常的子宫-胎盘血流导致新生儿体重下降,胎儿宫内发育迟缓、胎儿窘迫和新生儿Apgar评分降低。
Objective: To investigate the effect of fetus umbilical artery S / D ratio changes and perinatal outcome in patients with gestational hypertension. Methods: 63 cases of hypertensive disorder complicating pregnancy and 60 cases of primiparas without any surgical complications were randomly selected. Umbilical artery blood flow was measured in pregnant women with normal pregnancy and gestational hypertensive disorder. The umbilical artery S / D ratio and prognosis of perinatal relationship. Results: The gestational age and fetal weight of gestational hypertension group were lower than those of the normal group, and systolic blood pressure and diastolic blood pressure S / D ratio were higher than those of the normal group (P <0.01) Fetal umbilical artery S / D ratio higher than normal pregnancy group, the difference was statistically significant (P <0.01). The fetal umbilical artery S / D ratio was negatively correlated with fetal weight (P <0.01). CONCLUSIONS: Preterm eclampsia significantly reduced placental perfusion in preeclampsia, abnormal uterine-placental blood flow leading to decreased neonatal weight, delayed intrauterine growth, fetal distress, and neonatal Apgar scores.
