目的 研究溃疡性结肠炎 (UC)病人的白细胞介素 (IL) 1β、IL 1受体拮抗剂 (IL 1RA)、IL 4基因多态性 ,并分析其与抗中性粒细胞胞质抗体 (ANCA)及临床分型的关系。方法 用PCR 限制性片段长度多态性方法和序列特异性引物 PCR方法分别对 81例UC病人和 114例健康者进行IL 1β和IL 1RA、IL 4基因多态性分析。结果 UC组IL 4的RP2基因频率明显高于对照组 (2 9 0 %对 11 8% ,P =0 0 0 0 0 2 ) ,而对照组RP1基因频率明显高于UC组 (88 2 %对 71 0 % ,P =0 0 0 0 0 2 ) ,UC组RP1 2、RP2 2基因型的优势比值分别为 2 71(95 %CI,1 39~ 5 31) ,9 0 4 (95 %CI,1 0 5~ 2 0 3 2 9) ;2组IL 1β、IL 1RA各基因频率比较差异均无显著性 (P >0 0 5 ) ;ANCA(+)组和ANCA(- )组UC病人IL 4的RP1、RP2基因频率显著不同 ,差异有显著性 (P值均 <0 0 5 )。结论 中国汉族UC病人与IL 4内含子 3的基因多态性相关联 ,UC病人IL 4RP1基因频率明显降低 ,而RP2基因频率明显增加 ,与正常人的差异发生在ANCA(+)UC病人 ;中国汉族UC病人与IL 1β、IL 1RA基因多态性无关联。
Objective To study the polymorphisms of interleukin (IL) 1β, IL 1 receptor antagonist (IL 1RA) and IL 4 gene in patients with ulcerative colitis (UC) and analyze their relationship with anti-neutrophil cytoplasmic antibody ANCA) and clinical classification. Methods Polymorphisms of IL-1β, IL-1RA and IL-4 in 81 UC patients and 114 healthy controls were detected by PCR restriction fragment length polymorphism and sequence-specific primer PCR. Results The frequency of RP2 gene in IL 4 in UC group was significantly higher than that in control group (290% vs 118%, P = 0.00002), but the frequency of RP1 gene in control group was significantly higher than that in UC group (88.2% vs The odds ratios of RP1 2 and RP2 2 genotypes in UC group were 71% (95% CI, 139- 531), 94% (95% CI, 1 0 5 ~ 2 0 3 2 9). There were no significant differences in the frequencies of IL 1β and IL 1RA between the 2 groups (P> 0.05) The frequencies of RP1 and RP2 genes were significantly different (P <0.05). Conclusions The genotypes of IL 4 intron 3 are associated with the genetic polymorphism of IL 4 in Chinese Han patients. The frequency of IL 4 RP1 gene is significantly decreased in UC patients, while the frequency of RP2 gene is significantly increased. The difference from normal controls is in ANCA (+) UC patients. There was no association between IL-1β and IL-1RA gene polymorphisms in Chinese Han patients with UC.
