目的探讨有创与无创序贯机械通气治疗慢性阻塞性肺疾病(COPD)重症呼吸衰竭的疗效。方法接受气管插管和机械通气的患者44例,分为序贯治疗组和对照组进行研究。结果有创无创序贯治疗使呼吸频率明显下降、动脉收缩压降低、心率减缓、PSV水平降低以及PEEP降低,对改善呼吸肌疲劳和通气功能有一定意义。结论在临床工作中采用有创无创“序贯”机械通气治疗慢性阻塞性肺疾病合并呼吸衰竭能够比在PIC窗出现后仍按常规有创通气方法继续行机械通气具有更好的临床疗效。
Objective To investigate the curative effect of invasive and noninvasive sequential mechanical ventilation on severe respiratory failure in patients with chronic obstructive pulmonary disease (COPD). Methods 44 patients underwent tracheal intubation and mechanical ventilation were divided into sequential treatment group and control group. Results The invasive and sequential treatment significantly decreased respiratory rate, decreased systolic arterial pressure, decreased heart rate, decreased PSV and decreased PEEP, which was of some significance for improving respiratory muscle fatigue and ventilation. Conclusions The clinical application of invasive noninvasive sequential mechanical ventilation in the treatment of chronic obstructive pulmonary disease with respiratory failure can have better clinical efficacy than the conventional invasive ventilation after mechanical ventilation in the PIC window .
