目的:探讨单侧完全性唇腭裂(unilateral complete cleft lip and palate,UCLP)患者应用改良唇挡单侧扩弓后裂侧牙弓宽度变化的稳定性。方法:选取改良唇挡单侧扩弓与保持1年后的UCLP患者16例,男8例,性8例,平均年龄在13.3~14.5岁,对治疗前、扩弓治疗后及保持1年后的CBCT扫描图像进行测量,分别测量裂侧牙尖(颊尖)、牙槽基骨、根尖基骨至正中矢状面的距离,并进行统计学分析。结果:唇挡扩弓治疗后患者上颌裂侧(尖牙、第一、第二前磨牙)颊尖、牙槽基骨、根尖基骨至正中矢状面的距离均增加(P<0.05);保持1年后裂侧上颌颊尖、牙槽基骨至中线的距离较矫治结束时有所减小,根尖基骨至中线的距离少量增加。结论:UCLP患者经改良唇挡单侧扩弓,裂侧尖牙、第一、第二前磨牙牙尖、牙槽基骨、根尖基骨至正中矢状面的宽度均有显著增加,保持1年后扩弓效果稳定。
OBJECTIVE: To investigate the stability of dental arch width changes after unilateral complete cleft lip and palate (UCLP) unilateral amputation. Methods: Totally 16 UCLP patients were enrolled in this study. One hundred and twenty-eight male patients were enrolled in this study. The average age ranged from 13.3 to 14.5 years. After pre-treatment and post-expansion treatment and for one year The CBCT images were measured and the distances between cleft cusp (buccal cusps), alveolar bone and apical base bone to the median sagittal plane were measured and statistically analyzed. Results: The distance between maxillary fossa (canines, first and second premolar), buccal, alveolar bone and apical base to median sagittal plane were increased after lip-block expansion (P <0.05) After 1 year, the distance between maxillary buccal cusp and base of alveolar bone decreased from the end of orthodontic treatment, and the distance from apical base to midline increased a little. Conclusion: UCLP patients with unilateral expansion lip lip, fissured canine, the first and second premolar cusp, alveolar base bone, apical base to the median sagittal width was significantly increased, to maintain 1 year after the expansion of the effect of stability.
