目的:研究观察首乌丹参方预处理后对心肌缺血再灌注损伤(I/R)心肌梗死范围及血中白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)的影响,观察大鼠心肌诱生型一氧化氮合酶(iNOS)基因表达的变化,探讨首乌丹参方对心肌细胞的保护作用。方法:实验共设7个组,假手术组、缺血再灌注组(I/R组)和首乌丹参方高、中、低剂量组,对照药组(复方丹参滴丸、消心痛),采用结扎大鼠冠状动脉前降支30min/开放120min建立心肌I/R模型,观察I/R大鼠心肌梗死范围以及血中的IL-1β、TNF-α含量的变化;采用RT-PCR检测各组心肌iNOSmRNA的表达,同时电镜下观察心肌超微结构变化。结果:首乌丹参方高、中两个剂量组梗塞程度明显减轻,梗塞区重占全心脏及左心室的百分比与模型组比较均有显著性差异(P(0.01-0.001),以首乌丹参方中剂量组为优;首乌丹参方给药后血中TNF-α、IL-1β有不同程度的下降,其中以高、中剂量组为优(P(0.01);假手术组心肌iNOSmRNA弱表达,I/R组呈现高表达,首乌丹参方可下调iNOSmRNA基因表达;电镜下I/R组心肌组织严重损伤,首乌丹参方用药后可见局灶性损伤和毛细血管狭窄,心肌轻度损伤。结论:通过首乌丹参方预处理,可以减轻TNF-α、IL-1β对I/R心肌的损伤,下调I/R大鼠心肌iN-OSmRNA的表达,达到保护心肌的作用。
OBJECTIVE: To study the effect of Shouwu Danshenfang preconditioning on the myocardial infarct size, blood interleukin-1β and tumor necrosis factor-α (TNF-α) after myocardial ischemia-reperfusion injury (I/R) Observe the change of myocardial inducible nitric oxide synthase (iNOS) gene expression and investigate the protective effect of Shouwu Danshen prescription on cardiomyocytes. METHODS: A total of 7 groups were included in the study: sham operation group, ischemia-reperfusion group (I/R group), high-, middle-, and low-dose group of Shouwu Danshenfang group, and control group (Compound Danshen Dripping Pills and Xiaoxin Pain). Myocardial I/R model was established by ligation of the left anterior descending coronary artery for 30 min/120 min. The changes of myocardial infarct size, IL-1β and TNF-α in blood were observed in I/R rats. RT-PCR was used to detect the changes. The expression of iNOS mRNA was detected in myocardium, and the ultrastructure of myocardium was observed under electron microscope. RESULTS: The infarct level was significantly reduced in the two groups of Shouwu salvia miltiorrhiza, and the percentage of infarct zone reoccupying the whole heart and left ventricle was significantly different from that of the model group (P(0.01-0.001), Shouwu Danshen The dose in the middle-dose group was excellent. After administration of Shouwu Danshen prescription, the levels of TNF-α and IL-1β in blood decreased to varying degrees. Among them, the high and middle dose group was superior (P(0.01); the myocardial iNOS mRNA was weak in the sham operation group.
