目的:探讨醛固酮合酶基因单核苷酸多态性-344C/T与血管紧张素转换酶抑制剂(ACEI)降压疗效的相关性。方法:新发的轻中度原发性高血压患者121例,均来自北京市昌平地区的常住汉族人群,予以贝那普利口服治疗8w,利用TaqMan MGB等位基因分型试剂盒检测患者-344C/T基因多态性分布频率。结果:治疗后,TT基因型和CT基因型患者舒张压降低幅度分别为(13.50±4.91)mmHg(1mmHg=0.133 kPa)和(12.00±6.57)mmHg,高于CC基因型患者(8.64±6.74)mmHg,差异有统计学意义(P=0.045);经多元逐步回归分析,对可能影响ACEI类药物降压疗效的因素进行校正后,此多态性位点与治疗后舒张压的下降幅度有关(R2=0.052,P=0.023)。结论:醛固酮合酶基因-344C/T多态性可能与血管紧张素转换酶抑制剂的降压敏感性有关,T等位基因携带者服用贝那普利的降压效果更好。
Objective: To investigate the relationship between the single nucleotide polymorphism of aldosterone synthase gene -344C / T and antihypertensive efficacy of angiotensin-converting enzyme inhibitor (ACEI). Methods: A total of 121 new patients with mild-to-moderate essential hypertension were enrolled in this study. The patients were all from Han nationality in Changping, Beijing. The patients were treated with benazepril orally for 8 weeks. TaqMan MGB genotyping kit was used to detect patients- 344C / T gene polymorphism distribution frequency. Results: After treatment, the reduction of diastolic blood pressure was (13.50 ± 4.91) mmHg (1mmHg = 0.133 kPa) and (12.00 ± 6.57) mmHg in patients with TT genotype and CT genotype respectively, higher than those in CC genotype (8.64 ± 6.74) mmHg, the difference was statistically significant (P = 0.045). After multivariate stepwise regression analysis, the factors that may affect the antihypertensive effects of ACEIs were corrected, and the polymorphic sites were related to the decrease of diastolic blood pressure after treatment R2 = 0.052, P = 0.023). CONCLUSION: The polymorphism of aldosterone synthase gene -344C / T may be related to the antihypertensive sensitivity of angiotensin converting enzyme inhibitors, and the hypotensive effect of benazepril on T allele carriers is better.
