目的:研究降压治疗对老老年高血压患者动态血压节律的影响。方法:167例老老年高血压患者被随机分为降压治疗组(吲哒帕胺,或培托普利治疗)和安慰剂对照组,对比治疗1年。所有病例在安慰剂洗脱2个月后和治疗1年结束时,分别2次接受24 h动态血压监测(ABPM),并进行统计分析。结果:对照组和治疗组治疗前动态血压各指标比较均无显著性差异,节律为非杓型。治疗组治疗后SBP、SBP-L和PP显著降低(P<0.01),且较对照组显著降低(P<0.01);收缩压夜间下降率(SBPF)和舒张压夜间下降率(DBPF)分别为(5.8±1.98)%、(6.4±2.11)%,较对照组显著下降(P<0.05),动态血压节律呈浅杓型。结论:降压治疗对于减轻老老年高血压患者的靶器官损害可能有重要意义。
Objective: To study the effect of antihypertensive therapy on ambulatory blood pressure rhythm in elderly patients with hypertension. Methods: One hundred and sixty-seven elderly patients with hypertension were randomly divided into antihypertensive treatment group (Indapamide, Peitipril treatment) and placebo control group for one year. All cases received 24-h ambulatory blood pressure monitoring (ABPM) twice a month after placebo elution and at the end of 1 year of treatment, and statistical analysis was performed. Results: There was no significant difference in the indexes of ambulatory blood pressure between the control group and the treatment group before treatment, and the rhythm was non-dipper type. The SBP, SBP-L and PP in the treatment group were significantly decreased (P <0.01), and were significantly lower than those in the control group (P <0.01). The SBPF and DBPF were (5.8 ± 1.98)%, (6.4 ± 2.11)% respectively, which was significantly lower than that of the control group (P <0.05). The dynamic blood pressure rhythm showed a shallow dipper. Conclusion: Antihypertensive therapy may be of great significance in alleviating the target organ damage in elderly patients with hypertension.
