测定 5 4例 2型糖尿病患者及 2 5例健康对照者血清sVCAM -1和sICAM -1水平 ,2型糖尿病者按腓神经传导速度 (PNCV)分为神经病变组 ( 2 4例 )与无神经病变组 ( 3 0例 ) ,比较三组间sVCAM -1和sICAM -1的水平 ,并分析影响 2型糖尿病患者PNCV的因素。结果 :2型糖尿病患者血清sVCAM -1和sICAM -1的水平分别高于健康对照者 (P <0 0 0 1 ,P <0 0 5 ) ,有神经病变者sVCAM -1水平又高于无神经病变者 (P <0 0 5 ) ,多元逐步回归分析显示PNCV与病程、收缩压、sVCAM -1、空腹血糖呈独立负相关。结论 :sVCAM -1参与了糖尿病周围神经病变的发生发展过程 ,可作为周围神经病变病情监测的指标。
Serum levels of sVCAM-1 and sICAM-1 were measured in 54 patients with type 2 diabetes mellitus and 25 healthy controls. Patients with type 2 diabetes were divided into neuropathy group (24 cases) and no nerve group according to peroneal nerve conduction velocity (PNCV) Lesion group (30 cases). The levels of sVCAM-1 and sICAM-1 in the three groups were compared and the factors influencing PNCV in type 2 diabetic patients were analyzed. Results: Serum levels of sVCAM-1 and sICAM-1 in patients with type 2 diabetes mellitus were significantly higher than those in healthy controls (P <0.01 or P <0.05), and those with neuropathy were higher than those without neuropathy Multivariate stepwise regression analysis showed that PNCV was independently and inversely correlated with duration of disease, systolic blood pressure, sVCAM-1 and fasting blood glucose (P <0.05). Conclusion: sVCAM-1 is involved in the development and progression of diabetic peripheral neuropathy and may be used as an indicator of the condition of peripheral neuropathy.
