目的:探讨2型糖尿病肾脏早期损伤的实验室诊断指标。方法:分析92例2型糖尿病患者24小时尿液中的微量白蛋白(MALB/24H)、al-微球蛋白(α1MG/24H),尿N-乙酰-β-D氨基葡萄糖苷酶(NAG/24H)、尿肌肝(CR/24H)、尿尿素氮(BUN/24H)、尿尿酸(UA/24H)和血清胱抑素C(CYS-C)的水平。根据MALB/24H水平和临床蛋白尿及血肌酐将患者分为4组:无DN组、DNⅡ组、DNⅢ组、DNⅣ组。结果:DNⅡ组MALB/24H、α1MG/24H较无DN组高(P<0.05),并且下一组比上一组水平高(P<0.05),NAG/24H和CYS-CDNⅥ组水平比无DN组、DNⅡ组和DNⅢ组高(P<0.05),而CR/24H、BUN/24H和UA/24H各组间差别无统计学意义。联合检测有利于DN的早期诊断。“,”Objective Toclarify the value of urineMALB/24H,α1MG/24H ,NAG/24H,CR/24H,BUN/24H,UA/24H and serum CYS-C for therelrlydiagnasisof type 2DN.Methods The level of urineMALB/24H,α1MG/24H ,NAG/24H,CR/24H,BUN/24H,UA/24H and serum CYS-Cwer measured and analysed in 92 type 2 diabetic patients.the patient sweredevided to group without DN ,DNⅡ,DNⅢand group DNⅣaccording the level of Umalb/24H.compared the level of urineα1MG/24H ,NAG/24H,CR/24H,BUN/24H,UA/24H and serum CYS-C.Rsults The level of urineα1MG/24H in group DNⅡwere higher than that in group without DN(P<0.05),andthe level of next group were higher than that up group(P<0.05);the level of urine NAG/24H and CYS-C in group DNⅣwere higher than that in groupDNⅢ、DNⅡ、and group without DN.Conclusion The level of MALB/24H、α1MG/24H、NAG/24H and CYS-C may be indicaors for diagnosis of early type 2 DN. Combined detection can improve the positive rate, and is valuable of clinical diagnosis.
