目的探讨乳腺影像学报告及数据系统(breast imaging report and data system,BI-RADS)超声分级与超声弹性成像(ultrasonic elastograph,UE)在乳腺病灶良恶性鉴别上的价值。方法对415例女性患者共493个病灶术前行常规超声及弹性超声检查,采用BI-RADS超声分级、改良超声弹性评分及两者联合应用分别判断乳腺肿物的良恶性,以手术病理为诊断金标准,分析他们的诊断价值。结果 BI-RADS超声分级在鉴别乳腺良恶性病灶上的敏感性最高,达到94.0%,而BI-RADS超声分级与UE联合应用之后特异性、准确性和阳性预测值(positive predictive value,PPV)都显著提高(P<0.05)。ROC曲线下面积BI-RADS超声分级为0.896,UE为0.870,两者联合应用为0.903。结论两者联合应用的诊断价值要优于BI-RADS超声分级及UE。
Objective To investigate the value of ultrasonic grading and ultrasonic elastography (UE) in differentiating benign and malignant breast lesions from breast imaging report and data system (BI-RADS). Methods A total of 493 lesions of 415 female patients underwent routine ultrasound and elastography ultrasonography before surgery. BI-RADS classification, modified ultrasound elasticity score and the combination of the two methods were used to determine the benign and malignant tumor of breast respectively. The pathological diagnosis Gold standard, analyze their diagnostic value. Results The BI-RADS classification showed the highest sensitivity in differentiating benign and malignant breast lesions, reaching 94.0%, while the specificity, accuracy and positive predictive value (PPV) of BI-RADS were Significantly increased (P <0.05). The area under the ROC curve was 0.896 for BI-RADS and 0.870 for UE, and the combination of the two was 0.903. Conclusion The combined diagnostic value of the two methods is superior to BI-RADS ultrasound classification and UE.
