目的探讨三维数字成像在脑血管疾病诊断和治疗中的应用价值。方法应用GE Innova 3100平板DSA数字减影机。35例颅内动脉瘤患者,18例动静脉畸形患者,9例颈内动脉海绵窦漏。62例脑血管病病例行三维和二维DSA成像检查,应用二维数字减影血管造影患者的资料和应用三维数字减影血管造影患者的资料,对比分析三维和二维DSA曝光次数、造影剂的用量、患者接受的X光照射量和两种DSA图像特点。结果应用三维数字血管造影(3D-DSA)可以减少总曝光次数,不仅在选取参考图像时曝光次数减少,而且在手术过程中曝光次数也减少。两组造影剂的用量无统计学差异。3D-DSA图像因可以从三维角度观察而能得到更全面的信息。结论三维数字血管造影可以从多方位观察颅内血管的结构,可以提供更清晰的参考图像角度,从而可减少DSA的曝光数量,三维数字血管造影优于二维数字减影血管造影。
Objective To explore the value of three-dimensional digital imaging in the diagnosis and treatment of cerebrovascular diseases. Methods The GE Innova 3100 flat panel DSA digital subtraction camera was used. 35 patients with intracranial aneurysm, 18 patients with arteriovenous malformations, 9 cases of internal carotid artery cavernous leak. Sixty-two cases of cerebrovascular disease were examined by three-dimensional and two-dimensional DSA imaging. The data of two-dimensional digital subtraction angiography and the data of three-dimensional digital subtraction angiography were compared. Results Three-dimensional digital angiography (3D-DSA) can reduce the total number of exposures, not only reducing the number of exposures when selecting reference images, but also decreasing the number of exposures during the procedure. There was no significant difference in the amount of contrast medium between the two groups. 3D-DSA images can be more comprehensive information because of the three-dimensional perspective. Conclusion Three-dimensional digital angiography can observe the structures of intracranial vessels in different directions and provide a clearer reference image angle, which can reduce the number of DSA exposures. Three-dimensional digital angiography is superior to two-dimensional digital subtraction angiography.
