目的探讨癌胚抗原(CEA)蛋白、CEA mRNA的检测在大肠癌诊断中的价值。方法分别采用电化学发光免疫法和逆转录聚合酶链反应法检测42例大肠癌患者和45例健康对照者外周血CEA蛋白、CEA mRNA水平。结果大肠癌组CEA和CEA mRNA阳性率均明显高于正常对照组(P﹤0.05)。并且,Ⅲ~Ⅳ期大肠癌CEA和CEA mRNA阳性率明显高于Ⅰ~Ⅱ期(P﹤0.05)。CEA阳性率与大肠癌患者肿瘤大小和淋巴结转移无明显相关性(P﹥0.05)。CEA mRNA阳性率与大肠癌患者淋巴结转移明显相关(P﹤0.05),而与肿瘤大小无明显相关性(P﹥0.05)。结论血CEA蛋白、CEA mRNA在大肠癌患者中表达明显升高,其中CEA mRNA对于大肠癌的检测更为敏感,并且其表达与淋巴结转移密切相关,具有较好的临床诊断价值。
Objective To investigate the value of carcinoembryonic antigen (CEA) protein and CEA mRNA in the diagnosis of colorectal cancer. Methods The levels of CEA and CEA mRNA in peripheral blood of 42 colorectal cancer patients and 45 healthy controls were detected by chemiluminescence immunoassay and reverse transcription polymerase chain reaction, respectively. Results The positive rates of CEA and CEA mRNA in colorectal cancer group were significantly higher than those in normal control group (P <0.05). The positive rates of CEA and CEA mRNA in stage Ⅲ ~ Ⅳ colorectal cancer were significantly higher than those in stage Ⅰ ~ Ⅱ (P <0.05). There was no significant correlation between CEA positive rate and tumor size and lymph node metastasis in patients with colorectal cancer (P> 0.05). The positive rate of CEA mRNA was significantly correlated with lymph node metastasis in patients with colorectal cancer (P <0.05), but not with tumor size (P> 0.05). Conclusions The expression of CEA and CEA mRNA in blood of patients with colorectal cancer is obviously increased. CEA mRNA is more sensitive to the detection of colorectal cancer, and the expression of CEA mRNA is closely related to lymph node metastasis, which has a good clinical diagnostic value.
