目的探讨化疗期间肠外氨基酸营养支持治疗对老年非小细胞肺癌患者生活质量的影响。方法选择2012年2月至2014年8月间收治的60例老年非小细胞肺癌患者,根据随机数字表法分为观察组和对照组,每组30例。两组患者均行长春瑞滨+顺铂化疗方案,对照组化疗期间给予止吐、利尿等常规处理,观察组在对照组基础上给予肠外氨基酸营养支持治疗,比较两组患者治疗后的生活质量、不良反应及治疗前后营养状态变化。结果观察组和对照组患者治疗后躯体功能评分[(76.7±11.5)分和(67.2±15.4)分]、角色功能评分[(72.7±14.5)分和(65.2±13.4)分]、血清前白蛋白[(170.1±9.8)mg/L和(149.2±10.4)mg/L]、体质量指数[(17.7±2.6)kg/m2和(15.0±2.3)kg/m2]、血清白蛋白[(31.9±4.0)g/L和(26.5±3.3)g/L]的差异均有统计学意义(P<0.05)。观察组患者消化道反应、Ⅲ或Ⅳ级骨髓抑制发生率明显低于对照组,差异有统计学意义(P<0.05)。结论化疗期间肠外氨基酸营养支持治疗能有效改善肺癌患者营养状态,减少胃肠道反应等不良反应,提高患者生活质量。
Objective To investigate the effect of parenteral amino acid nutrition support on quality of life in elderly patients with non-small cell lung cancer during chemotherapy. Methods Sixty elder non-small cell lung cancer patients who were admitted between February 2012 and August 2014 were randomly divided into observation group and control group according to the random number table method, with 30 cases in each group. The two groups of patients were vinorelbine + cisplatin chemotherapy, the control group during chemotherapy given antiemetic, diuretic and other conventional treatment, the observation group on the basis of the control group given parenteral amino acid nutrition support treatment, compared two groups of patients after treatment of life Quality, adverse reactions and changes in nutritional status before and after treatment. Results Body function scores [(76.7 ± 11.5) and (67.2 ± 15.4) points], role function scores [(72.7 ± 14.5) points and (65.2 ± 13.4) points, (170.1 ± 9.8) mg / L and (149.2 ± 10.4) mg / L], body mass index [(17.7 ± 2.6) kg / m2 and 15.0 ± 2.3 kg / m2], serum albumin ± 4.0) g / L and (26.5 ± 3.3) g / L] (P <0.05). The observation group patients with gastrointestinal reactions, Ⅲ or Ⅳ grade bone marrow suppression was significantly lower than the control group, the difference was statistically significant (P <0.05). Conclusion The parenteral amino acid nutrition support therapy during chemotherapy can effectively improve the nutritional status of patients with lung cancer, reduce gastrointestinal reactions and other adverse reactions and improve the quality of life of patients.
