早期心肌缺血预适应( I P C) 对心脏具有显著的保护作用,但这种保护作用在 I P C 后1 ~2h即消失。近年来发现, I P C 所致的心脏保护作用在 I P C 后24h 又可重现,即第二保护窗( S W P) 。与早期 I P C 相比, S W P 至少具有二个特点:时间框宽和所需 I P C 刺激强。 S W P 的发生机理与早期 I P C 不完全相同,前者主要与热休克蛋白表达增加和内源性抗氧化因子增加有关。可以预料 S W P 的临床应用前景肯定比早期 I P C 更具吸引力。
Early myocardial ischemic preconditioning (I P C) has a significant protective effect on the heart, but this protective effect disappears 1 to 2 h after I P C. In recent years, it has been found that the cardioprotective effect of I P C can be reproduced 24 h after I P C, ie, the second protective window (S W P). S W P has at least two traits compared to early I P C: a long time frame and a strong I P C stimulus. The mechanism of S W P is not exactly the same as the early I P C. The former is mainly related to the increase of heat shock protein and the increase of endogenous anti-oxidant. It is anticipated that the clinical application of SWP will certainly be more attractive than the early IPCs.
