目的探讨血小板激活因子受体拮抗剂苦杏内酯AB(GAB)对急性胰腺炎(AP)鼠血流变学、胰腺黏附分子表达及胰、胰外器官损害的影响。方法 SD大鼠90只随机分为假手术组 (A组,30只)、AP组(B组,30只)AP加GAB治疗组(C组,30只),观察各组血流变学、酶学、胰腺组织黏附分子E-选择素(E-selectin)、细胞间黏附分子(ICAM)-1表达、胰及肺、肠形态学改变。结果 AP组,低切下全血黏度9.41±0.80、红细胞聚集指数2.38±0.10、红细胞刚性指数4.88± 0.18,胰黏附分子表达评分E-selectin 2.2、ICAM-I 2.3,胰及肺、肠评分或分级病损严重;治疗组上述各血流变学指标依次为7.41±0.51、2.19±0.11、3.98±0.10(与AP组比P<0.01),胰腺组织E- selectin 1.1、ICAM-1 1.1C(与AP组比P<0.01),胰及肺、肠胰外器官病损减轻(P<0.01)。结论 AP时,黏附分子异常表达及血流变异常是导致胰、胰外器官损害的重要病理生理基础,早期应用 PAF受体拮抗剂GAB能有效改善AP血微循环障碍、血流变异常及降低黏附分子表达,对胰及胰外器官损害有保护作用。
Objective To investigate the effect of platelet activating factor receptor antagonist, bitter algalactone AB (GAB) on hemorheology, expression of pancreatic adhesion molecules and pancreatic and pancreatic organ damage in acute pancreatitis (AP) rats. Methods Ninety Sprague-Dawley rats were randomly divided into sham operation group (A group, 30 rats), AP group (B group, 30 rats), AP plus GAB treatment group (C group, 30 rats) Enzymeology, E-selectin, ICAM-1 expression, pancreatic, lung and intestinal morphological changes were observed. Results In the AP group, the whole blood viscosity at low cut-off was 9.41 ± 0.80, the erythrocyte aggregation index was 2.38 ± 0.10, the erythrocyte rigidity index was 4.88 ± 0.18, and the expression of pancreatic adhesion molecule was E-selectin 2. 2, ICAM-I 2.3, pancreatic and lung, intestine score or grading serious lesions; the treatment group, the above indicators of blood rheology followed by 7.41 ± 0.51,2.19 ± 0.11,3. 98 ± 0.10 (P <0.01 compared with AP group), E-selectin 1.1, ICAM-1 1.1C (P <0.01) Lesions in external organs were alleviated (P <0.01). Conclusions Abnormal expression of adhesion molecules and abnormal blood flow in AP are the pathophysiological basis of organ damage in pancreas and pancreas. Early application of GABA, a PAF receptor antagonist, can effectively improve microcirculation disturbance, abnormal blood flow and decrease of blood flow in AP Adhesion molecule expression, pancreatic and pancreatic organ damage have a protective effect.
