目的提高巨大儿的产前诊断,减少母儿分娩时并发症。方法对160例巨大儿临床资料进行回顾性分析。结果巨大儿发生率为3.68%,有逐年上升趋势;发病因素除与遗传、糖尿病、过期妊娠有关外,还与孕妇营养因素密切相关;巨大儿在分娩中母儿并发症明显高于非巨大儿组(P<0.01),产前综合预测率达86%。结论产前单项指标预测率均不高,产前除做好糖尿病筛查外,还应综合孕妇体重、宫高、腹围及B超检查(双顶径>96mm、股骨长度>72mm、腹径>100mm)多项指标及时诊断;适当放松剖宫产指征是降低巨大儿分娩并发症的关键。
Objective To improve the prenatal diagnosis of giant children and reduce complications during childbirth. Methods The clinical data of 160 cases of giant children were analyzed retrospectively. Results The incidence of macrosomia in children was 3.68%, which showed an upward trend year by year. In addition to genetic factors, diabetes, and overdue pregnancy, the incidence of macrosomia was also closely related to the nutritional factors of pregnant women. The morbidity of maternal children in macrosomia was significantly higher than that of non- Huge children group (P <0.01), prenatal comprehensive prediction rate of 86%. Conclusion The prenatal single index predictive rate is not high, in addition to good prenatal diabetes screening, but also comprehensive weight, height, abdominal circumference and B-ultrasound (biparietal diameter> 96mm, femur length> 72mm, > 100mm) timely diagnosis of a number of indicators; appropriate relaxation of indications for cesarean delivery is to reduce the huge childbirth complications of the key.
