目的探讨2型糖尿病患者纤维蛋白原(FIB)水平与尿白蛋白排泄率(UAER)及内生肌酐清除率(Ccr)之间的关系。方法选取2型糖尿病患者268例,根据UAER不同分为尿蛋白量正常组(A组)、微量蛋白尿组(B组)、大量蛋白尿组(C组),分析各组间FIB水平与生化指标、Ccr之间的关系。结果 B、C 2组的糖化血红蛋白(Hb Alc)、空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、血肌酐(Scr)、FIB水平高于A组,差异有统计学意义(P<0.05);Ccr明显低于A组,差异有统计学意义(P<0.05);高FIB血症发生率为C组>B组>A组,且差异有统计学意义(P<0.05)。异常UAER(B组+C组)患者的UAER与Hb A1c、FPG、TC、TG、Scr、FIB水平呈正相关(P<0.05);Ccr与UAER、FPG、Scr、FIB水平呈负相关(P<0.05);Logistic逐步回归分析结果提示,FIB是预测UAER的独立危险因素。结论 2型糖尿病患者FIB水平与UAER和肾功能下降的程度有独立相关性,可以作为UAER和肾功能损害程度的预测因子。
Objective To investigate the relationship between fibrinogen (FIB) level and urinary albumin excretion rate (UAER) and endogenous creatinine clearance (Ccr) in type 2 diabetic patients. Methods A total of 268 patients with type 2 diabetes mellitus (T2DM) were enrolled in this study. According to the UAER, they were divided into normal group (group A), microalbuminuria group (group B) and massive proteinuria group (group C) Indicator, the relationship between Ccr. Results The blood glucose of Hb Alc, fasting blood glucose (FPG), total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), serum creatinine (Scr) (P <0.05); Ccr was significantly lower than that of A group (P <0.05); the incidence of hyperlipidemia was C> B> A Group, and the difference was statistically significant (P <0.05). There was a positive correlation between UAER and levels of Hb A1c, FPG, TC, TG, Scr and FIB in patients with abnormal UAER (group B + C) (P <0.05) 0.05). Logistic stepwise regression analysis indicated that FIB was an independent risk factor for UAER. Conclusions The level of FIB in patients with type 2 diabetes has an independent correlation with the degree of decline of renal function and UAER, which may be used as a predictor of the degree of renal damage.
