目的 观察新型人工颈椎间盘假体置入犬颈椎后能否维持颈椎生理曲度和椎间隙高度,假体能否保持适度的活动能力及假体与骨的初期和长期稳定性.方法 参照对15 条犬颈椎的几何测量,设计新型犬用羟基磷灰石(hydroxyapatite,HA) 涂层人工C6- 7 间盘假体.20 条成年犬随机分为3 、6、12 周HA 涂层组及6 周无涂层组.行X 线摄片、组织学观察、生物力学及力学推出试验、组织形态计量学测定.结果 X 线显示术后各组犬颈椎生理曲度正常,C6 -7 置换椎间隙高度正常.HA涂层假体与周围骨紧密结合,而3 例无涂层假体与骨之间可见透亮线.组织学观察见术后各期HA 涂层均与骨直接紧密结合,无涂层假体与骨间存在间隙.生物力学实验示C6- 7 假体置入术后各组较置入即刻的各向活动范围减小.置入术后各组之间各向活动范围差别无显著性意义.置入术后12 周活动范围为前屈5 .92°±2.12°,后伸6.65°±2 .18°,右侧弯7.58°±1 .56°,左侧弯7.50°±1 .69°,与相邻C5,6 椎间隙各组各向活动范围差别无显著性意义.力学推出试验与组织计量学示各HA 组剪切强度及骨结合率分别为:3 周(1.09 ±0.41)MPa,(?“,”Objective To observe the maintainance for lordosis and height of intervertebral space of the cervical spine, the motive ability of prosthesis,and the current and long term stability of prosthesis with bone after implantation of a new type of cervical intervertebral disk prosthesis (CIDP) into canine C 6-7 spine. Methods CIDP was designed according to the geometric measurement of C 6-7 intervertebral disk in 15 dogs. Totally 20 adult dogs were randomly divided into 4 groups: HA coated 3,6,12 weeks group and none HA coated 6 weeks group. Radiography, histological study,biomechanical test, push out test, histomorphometry were used in this study. Results X-ray examination showed the lordosis and the C 6-7 space are normal,there is a close connection between HA coated CIDP and bone,radiolucent line could be observed in 3 cases of none HA coated CIDP. Histologically, there is a close connection between bone and HA coated CIDP, a clear space is observed between bone and none HA coated CIDP. There is a significant decrease in the range of motion (ROM)in each implanted group compared to immediately replaced group. There is no significant difference between individual prosthesis replaced group. The ROM of 12 weeks group is: flexion 5.92°±2.12°, extention 6.65°±2.18°,right bending 7.58°±1.56°,left bending 7.50°±1.69°. There is no change in ROM of C 5-6 during the experiment period. The shear strength and bone cover rate of HA group increased after operation: the actual data were (1.09±0.41) MPa,(42.20±2.60)%,(2.04±0.34) MPa,(74.00±1.20)%;(2.59±0.07)MPa,(69.50±20.00)%,at 3,6,12 weeks after surgery respectively, significant higher than that in the none HA coated group(0.29±0.15)MPa,(6.40±8.90)%. Conclusion HA coated CIDP is fairly good in maintaining the lordosis and the space height of cervical spine,as well as stable and suitable ROM;it could gain current and long term reliable fixation with the vertebral bone, and possesses possibility of clinical use.
