目的探讨瘦素对胎儿生长发育的影响及其与代谢类激素的关系。方法采用放射免疫分析法对河北医科大学第一医院2003~2004年出生的83例足月新生儿进行瘦素、胰岛素(RI)、生长激素(GH)、促甲状腺激素(TSH)、甲状腺激素(T3、T4)测定,测量新生儿出生体重、身长、记录性别及分娩方式并计算体重指数(BMI),评估新生儿营养状态。结果(1)足月新生儿脐血瘦素质量浓度为(10.53±5.04)μg/L,性别间差异无显著性(P>0.05);不同分娩方式间差异无显著性(P>0.05)。(2)新生儿脐血瘦素质量浓度与体重指数呈正相关(r=0.5196,P<0.01)。瘦素质量浓度在大于胎龄(LGA)儿、适于胎龄(AGA)儿、小于胎龄(SGA)儿3组间差异有显著性(P<0.01),AGA儿组明显低于LGA儿组而高于SGA儿组。(3)新生儿脐血瘦素质量浓度与GH呈正相关(r=0.5946,P<0.01),与RI呈正相关(r=0.4364,P<0.05),与TSH呈负相关(r=-0.4268,P<0.05),与总T3、总T4呈正相关(r分别为0.4472、0.3571,P均<0.05)。结论瘦素对胎儿宫内生长发育起重要的调节作用,与代谢类激素关系密切。
Objective To investigate the effect of leptin on fetal growth and development and its relationship with metabolic hormones. Methods Eighty-three full-term newborns born from 2003 to 2004 in the First Hospital of Hebei Medical University underwent radioimmunoassay. The levels of leptin, insulin (RI), growth hormone (GH), thyrotropin (TSH), thyroid hormone T3 and T4). The newborn’s birth weight, length, sex and mode of delivery were measured and the body mass index (BMI) was calculated to assess the nutritional status of the newborn. Results (1) The cord blood leptin concentration of full-term neonates was (10.53 ± 5.04) μg / L with no significant difference between sexes (P> 0.05). There was no significant difference between different delivery modes (P> 0.05). (2) Neonatal cord blood leptin concentration and body mass index were positively correlated (r = 0.5196, P <0.01). Leptin concentration was significantly higher in LGA children than in AGA children and AGGA children in SAG group (P <0.01) Group and higher than the SGA group. (3) Neonatal cord blood leptin concentration was positively correlated with GH (r = 0.5946, P <0.01), positively correlated with RI (r = 0.4364, P < P <0.05). There was a positive correlation between total T3 and total T4 (r = 0.4472, 0.3571, P <0.05, respectively). Conclusion Leptin plays an important regulatory role in intrauterine growth and development, and is closely related to metabolic hormones.
