目的:探讨大鼠重症急性胰腺炎(SAP)肺损伤中NF-κB活化与细胞凋亡的关系。方法:选择Wistar大鼠共72只,随机分为假手术组、SAP模型组(模型组)、NF-κB抑制剂二硫代氨基甲酸吡咯烷(PDTC)预处理+SAP模型组(PDTC组),各组分别在造模后2、6、8、12 h取6只大鼠,收集肺组织光镜下观察病理变化,利用免疫组织化学法检测NF-κB的表达,检测细胞凋亡。结果:除假手术组外,其余两组大鼠造模后均出现明显的肺损伤,且随时间推移逐渐加重,但PDTC组在各时间点肺损伤程度低于模型组;假手术组大鼠肺组织有极少量的NF-κB表达与凋亡细胞,而其余两组造模后肺组织均有明显的NF-κB表达与细胞凋亡,且均在造模后6 h达高峰,但PDTC组NF-κB表达量及凋亡细胞数均明显少于模型组,差异均有统计学意义(均P<0.05);相关性分析显示,SAP大鼠肺组织NF-κB活性与细胞凋亡率呈明显正相关(3 h时,r=0.93,P=0.02;6 h时,r=0.95,P=0.021;8 h时,r=0.82,P=0.038;12 h时,r=0.98,P=0.02)。结论:大鼠SAP肺损伤中的细胞凋亡增加可能与NF-κB活性升高有关。
Objective: To investigate the relationship between activation of NF-κB and apoptosis in lung injury of severe acute pancreatitis (SAP) in rats. Methods: A total of 72 Wistar rats were randomly divided into three groups: sham operation group, SAP model group (model group), NF-κB inhibitor PDTC preconditioning + SAP model group (PDTC group) The rats in each group were sacrificed at 2, 6, 8, and 12 h after modeling respectively. The pathological changes of the lungs were observed under light microscope. The expression of NF-κB was detected by immunohistochemistry and the apoptosis was detected. Results: In addition to the sham operation group, the other two groups of rats showed obvious lung injury after modeling, and gradually increased with the passage of time. However, the degree of lung injury in PDTC group was lower than that of the model group at each time point. There was a very small amount of NF-κB expression in lung tissue and apoptotic cells, while the other two groups showed significant NF-κB expression and apoptosis in the lung tissue, all at 6 h after modeling, but PDTC The expression of NF-κB and the number of apoptotic cells in the model group were significantly lower than those in the model group (all P <0.05). The correlation analysis showed that the NF-κB activity and the apoptosis rate (R = 0.93, P = 0.02 at 3 h, r = 0.95, P = 0.021 at 6 h, r = 0.82, P = 0.038 at 8 h, r = 0.98, P = 0.02). Conclusion: The increased apoptosis in lung injury of SAP may be related to the increase of NF-κB activity.
