目的探讨急性左心功能衰竭(ALHF)和充血性心力衰竭(CHF)患者肝功能检测指标的差异。方法选取我院收治的心力衰竭患者120例,根据病因将其分成两组,即ALHF组56例,CHF组64例,分别对两组的肝功能进行检测,对比其肝功能指标的差异和病死率。结果 ALHF组的谷丙转氨酶、谷草转氨酶、白蛋白含量均明显高于CHF组(P<0.05),而直接胆红素、间接胆红素、谷氨酰氨基转移酶含量却明显低于CHF组(P<0.05);住院治疗期间,ALHF组病死率为26.79%(15例),CHF组为9.38%(6例),两组比较有统计学差异(P<0.05)。结论 ALHF患者的谷草转氨酶、谷丙转氨酶、白蛋白含量显著升高,由于肝脏严重缺血所致;而CHF患者的直接/间接胆红素、谷氨酰氨基转移酶含量升高,主要由于肝脏淤血所致。
Objective To investigate the difference of liver function tests between patients with acute left heart failure (ALHF) and congestive heart failure (CHF). Methods A total of 120 patients with heart failure admitted to our hospital were divided into two groups according to their etiology: 56 cases in ALHF group and 64 cases in CHF group. The liver function of the two groups were detected respectively, and their differences in liver function and mortality were compared rate. Results The levels of ALT, AST and ALB in ALHF group were significantly higher than those in CHF group (P <0.05), while the levels of direct bilirubin, indirect bilirubin and glutaminyl transaminase in ALHF group were significantly lower than those in CHF group (P <0.05). During hospitalization, the mortality rate of ALHF group was 26.79% (15 cases) and that of CHF group was 9.38% (6 cases). There was significant difference between the two groups (P <0.05). CONCLUSIONS: Aspartate aminotransferase, alanine aminotransferase, and albumin levels are significantly elevated in patients with ALHF due to severe hepatic ischemia, whereas the levels of direct / indirect bilirubin and transglutaminase are elevated in patients with CHF, mainly due to liver Caused by congestion.
