目的研究子宫内膜异位症患者辅助性T淋巴细胞(Th)亚群的变化及在子宫内膜发病中的作用。方法选取该院妇产科确诊的40例子宫内膜异位症行腹腔镜手术的妇女(内异症组)和同期行输卵管结扎或吻合术的30例盆腔结果正常者作为对照组,采用酶联免疫吸附法(ELISA)分别检测两组研究对象的血清、腹腔液中的Th1型细胞因子γ干扰素(IFN-γ)、白细胞介素-2(IL-2),Th2型细胞因子白细胞介素-4(IL-4)、白细胞介素-6(IL-6)的水平差异。结果内异症组血清中的IL-4(32.95±7.36)ng/L、IL-6(5.37±2.96)ng/L、腹腔液中的IL-4(17.96±5.13)ng/L、IL-6(2.24±0.95)ng/L均显著地高于对照组血清中IL-4(24.51±6.94)ng/L、IL-6(2.80±1.14)ng/L、腹腔液中IL-4(11.42±4.98)ng/L、IL-6(1.17±0.72)ng/L,且差异均具有统计学意义(P<0.05)。子宫内膜异位症Ⅰ+Ⅱ期血清中的IL-4(27.84±7.59)ng/L、IL-6(3.96±1.25)ng/L、腹腔液中IL-4(14.26±4.19)ng/L、IL-6(1.67±0.64)ng/L均显著地低于III+IV期患者血清中IL-4(34.38±8.05)ng/L、IL-6(6.27±2.54)ng/L、腹腔液中IL-4(19.85±5.03)ng/L、IL-6(2.40±0.82)ng/L,且差异均具有统计学意义(P<0.05)。结论子宫内膜异位症患者血清及腹腔液中的IL-4、IL-6水平发生显著的变化,提示Th2细胞比例发生变化,患者的免疫水平发生变化可能在内异症发生、发展中具有重要作用。
Objective To study the changes of helper T lymphocyte subsets and their roles in the pathogenesis of endometriosis in patients with endometriosis. Methods Forty women with endometriosis (endometriosis group) diagnosed as obstetrics and gynecology and 30 normal pelvic masses with tubal ligation or anastomosis during the same period were selected as the control group. The levels of Th1 cytokine interferon (IFN-γ), interleukin-2 (IL-2), Th2 cytokine interleukin 4 (IL-4), interleukin-6 (IL-6) level differences. Results Serum levels of IL-4 (32.95 ± 7.36) ng / L, IL-6 (5.37 ± 2.96) ng / L, IL-4 in peritoneal fluid (17.96 ± 5.13) ng / (2.24 ± 0.95) ng / L were significantly higher than those in control group (24.51 ± 6.94 ng / L, 2.80 ± 1.14 ng / L, IL-4 ± 4.98) ng / L and IL-6 (1.17 ± 0.72) ng / L, respectively, with statistical significance (P <0.05). Conclusions The levels of IL-4 and IL-6 in serum and peritoneal fluid of patients with endometriosis have significant changes, which indicates that the proportion of Th2 cells changes.
