目的:探讨沙丁胺醇对无症状支气管哮喘患者舒张试验评定标准的有关指标。方法:对无症状哮喘患者雾化吸入沙丁胺醇200μg,10~20min前后进行各项肺功能测试,按FEV1>15%基础值,最大用力呼气流量曲线FEF各项值>20%基础值为阳性标准。结果:按肺功能分级严重程度,其阳性率:轻度组FEV1仅为8.8%,中度组FEV1为31.6%,FEF25%~75%为42.1%,而在Vmax50与Vmax25则分别为52.6%;只有在重度组FEV1为51.6%,FEF25%~75%、Vmax50、Vmax25分别为64.5%、64.5%和58.1%。结论:如仅以FEV1>基础值15%为阳性诊断标准,将有较多经临床诊断为哮喘的患者达不到此标准,特别对轻度和中度无症状哮喘患者应综合考虑FEF25%~75%、Vmax50、Vmax25等指标的变化,以对本试验的评价更为客观。
Objective: To investigate the relevant indexes of salbutamol for evaluating the diastolic test in patients with asymptomatic bronchial asthma. Methods: Aspirin asthma patients inhaled salbutamol 200μg, 10 ~ 20min before and after the lung function tests, FEV1> 15% of the baseline values, the maximum forced expiratory flow curve FEF> 20% of the value of the standard value of the positive standard . Results: The positive rate of FEV1 was only 8.8% in mild group, 31.6% in moderate group, and 42.1% in FEF25% ~ 75%, according to the grading of pulmonary function. CONCLUSIONS: If only 15% of patients with FEV1> basal is considered a positive diagnostic criteria, more patients with clinically diagnosed asthma will not reach this standard. In particular, patients with mild and moderate asymptomatic asthma should consider FEF25% 75%, Vmax50, Vmax25 and other indicators of changes in order to evaluate the test more objective.
