目的:介绍一种新的心肌梗死模型制作方法,为心肌梗死的中长期研究及完成一些疑难的、需要反复进胸的心肌梗死实验提供良好的平台。方法:取8~11个月龄健康家猪,体质量30 kg左右,氯胺酮、吗啡麻醉,气管插管,呼吸气囊辅助呼吸给氧。取右侧斜卧位,胸骨左缘第3肋间切口长约3.0 cm,于上一肋间腋前线处取一约1.5 cm切口置光源。进胸后在电视胸腔镜辅助下显露钝缘支,5-0 prolene无创缝扎,术毕鼓肺排气拔除胸引管,同时关胸。主要观察指标:心肌酶谱、肌钙蛋白、心电图,同时标本进行HE染色。结果:该方法可以完成急性、亚急性、慢性心肌梗死模型建立,结果可靠,重复性好,模型猪能长期存活,生存率高,术后易于长期饲养。结论:在电视胸腔镜辅助下能更好地完成猪心肌梗死模型。
OBJECTIVE: To introduce a new method for making a myocardial infarction model, providing a good platform for mid-term and long-term studies of myocardial infarction and for accomplishing some difficult and demanding myocardial infarction experiments. Methods: Healthy pigs aged from 8 to 11 months were given 30 kg body weight. Ketamine, morphine anesthesia, endotracheal intubation and respiratory balloon assisted breathing. Take the right oblique position, the left sternal border of the 3rd intercostal incision about 3.0 cm, on the anterior intercostal axillary line to take a about 1.5 cm incision set light source. Into the chest after the help of video-assisted thoracoscopic blunt marginal branch, 5-0 prolene non-invasive sutures, complete removal of the lungs and lung surgery chest catheter, and off the chest. MAIN OUTCOME MEASURES: Myocardial zymogram, troponin, electrocardiogram, simultaneous HE staining of specimens. Results: This method can establish acute, subacute and chronic myocardial infarction models with reliable results and good reproducibility. The model pigs can survive long-term, with high survival rate and long-term post-operative feeding. Conclusion: Piglet myocardial infarction model can be completed better with video-assisted thoracoscopy.
