【目的】观察四磨汤对便秘型肠易激综合征(IBS-C)的干预作用及可能的机制。【方法】选用SPF级SD大鼠,将其随机分为正常组,模型组,中药高、中、低剂量组(剂量分别为3.6、1.8、0.9 g·kg-1·d-1),阳性对照组(马来酸曲美布汀,剂量为0.1 g·kg-1·d-1);采用夹尾激怒联合直肠刺激法复制大鼠IBS-C模型,给药后测定各组大鼠下丘脑、脑干部位的肾上腺皮质激素释放因子(corticotropin-releasing hormone,CRH)水平,腰段脊髓P物质(substance P,SP)含量,肠黏膜肥大细胞活化状态(c-fos基因表达情况)。【结果】经四磨汤治疗2周后,与模型组比较,中药高、中、低剂量组大鼠内脏高敏感性均显著降低(P<0.05)。与正常组比较,模型组大鼠脑干和下丘脑部位的CRH含量显著升高,脊髓SP含量显著降低,肠黏膜c-fos基因表达总阳性显色细胞数显著降低,差异均有统计学意义(P<0.05);与模型组比较,阳性对照组,中药高、中、低剂量组大鼠脑干和下丘脑CRH水平均显著降低,大鼠脊髓SP含量显著升高,肠黏膜c-fos基因表达总阳性显色细胞数显著升高,差异均有统计学意义(P<0.05)。【结论】四磨汤治疗IBS-C的作用与其降低大鼠脑干、下丘脑CRH含量,升高脊髓P物质含量与c-fos基因表达水平,影响脑—肠轴通路有关。
【Objective】 To observe the intervention effect of Si mill decoction on constipation-induced irritable bowel syndrome (IBS-C) and its possible mechanism. 【Methods】 SD rats of SPF grade were randomly divided into normal group, model group, high, medium and low dose groups (doses of 3.6, 1.8, 0.9 g · kg-1 · d-1) The control group (trimebutine maleate at a dose of 0.1 g · kg-1 · d-1); IBS-C model was duplicated by tinea capitis and rectal irritation; after administration, The levels of corticotropin-releasing hormone (CRH), substance P (SP) in the lumbar spinal cord and the activation status of intestinal mucosal mast cells (c-fos gene expression) in the thalamus and brain stem areas were measured. 【Results】 After two weeks of treatment with Sihuangtang, the visceral hypersensitivity of rats in high, medium and low dose groups were significantly decreased compared with model group (P <0.05). Compared with the normal group, the content of CRH in the brainstem and hypothalamus of the model group was significantly increased, the content of SP in the spinal cord was significantly decreased, the total number of positive cells in c-fos gene expression was significantly decreased, the differences were statistically significant (P <0.05) .Compared with the model group, the levels of CRH in the brainstem and hypothalamus of the rats in the positive control group and the TCM groups were significantly decreased, while the content of SP in the spinal cord of the rats in the positive control group and the traditional Chinese medicine group was significantly increased. The c-fos The total number of gene expression positive cells was significantly increased, the difference was statistically significant (P <0.05). 【Conclusion】 Siriang Decoction can reduce the level of CRH in rat brainstem and hypothalamus, increase the content of substance P in the spinal cord and the expression of c-fos gene, and affect the path of brain-gut axis.
