目的调查医院门诊处方退药情况,统计分析退药原因,减少退药情况发生。方法收集2006年7~12月门诊退药处方共计60份,按退药原因分类。结果药物不良反应是退药的主要原因,医师药师因素、患者因素也是重要原因。结论加强医师诊疗中的问诊和告知义务,提高药师审核调配处方能力,向患者多作解释工作,减少日常差错发生,加强医师、药师之间的沟通,加强药品说明书的学习,可减少门诊退药的发生。
Objective To investigate the withdrawal status of outpatient prescriptions in hospitals, analyze the reasons of withdrawal and reduce the withdrawal of drugs. Methods A total of 60 prescriptions for outpatient withdrawal from July to December 2006 were collected and categorized according to the reason of withdrawal. Results The adverse drug reaction was the main reason for the withdrawal. The physician-physician factor and patient factor were also the important reasons. Conclusion To strengthen the consultation and informing obligations of physicians in diagnosis and treatment, improve the ability of pharmacists to examine and allocate prescriptions, explain more to patients, reduce the occurrence of daily errors, strengthen the communication between physicians and pharmacists and strengthen the instruction of drug prescriptions, thus reducing outpatient visits Drugs happen.
