目的探讨对肿瘤骨转移患者进行个体化营养支持疗法的效果及其效价性,为个体化营养疗法的临床应用和推广提供相关支持资料。方法选取湖南省肿瘤医院2014年9月-2016年10月收治的100例肿瘤骨转移患者作为研究对象,随机分为对照组和观察组(每组50例),所有患者均接受4周的营养治疗,对照组给予常规营养支持治疗,治疗组则给予个体化营养支持治疗。比较两组治疗前后的体重、营养及生化指标、生活质量、营养治疗费用、住院天数以及患者对治疗方案的满意度。结果治疗前,两组患者的体重、生化指标、营养危险指数(NRI)、预后营养指数(PNI)以及Karnofsky生活质量评定表(KPS)评分差异均无统计学意义(P>0.05)。治疗后,对照组上述指标与治疗前差异无统计学意义(P>0.05);而治疗组的指标较治疗前有显著改善,且明显优于对照组(P<0.05)。治疗后,治疗组的营养治疗费用为(1 024.9±158.7)元,住院天数为(25.5±4.2)d,均明显少于对照组的(4 760.1±245.6)元和(30.1±5.6)d(P<0.05)。治疗组患者满意度为96%,明显高于对照组的78%(P<0.05)。结论对肿瘤骨转移患者进行个体化营养支持疗法,能明显提高患者的预后营养指数,显著降低患者的营养不良风险,大幅减少患者的住院费用和住院天数,且可提升患者满意度,有效改善医患关系,值得临床推广应用。
Objective To explore the effect and potency of individualized nutrition supportive therapy for patients with tumor bone metastasis and to provide relevant supporting information for the clinical application and promotion of individualized nutrition therapy. Methods 100 patients with tumor bone metastases from September 2014 to October 2016 in Hunan Cancer Hospital were randomly divided into control group and observation group (50 cases in each group). All patients received nutrition for 4 weeks The patients in the control group were given routine nutritional support and the patients in the treatment group were given personalized nutritional support. Before and after treatment, the body weight, nutrition and biochemical indexes, quality of life, cost of nutrition treatment, days of hospitalization and patients’ satisfaction with treatment regimen were compared. Results There was no significant difference in weight, biochemical, NRI, PNI and Karnofsky scores before and after treatment (P> 0.05). After treatment, the above indicators in the control group were not significantly different from those before treatment (P> 0.05), while those in the treatment group were significantly improved compared with those before treatment (P <0.05). After treatment, the cost of nutrition treatment in the treatment group was (1 024.9 ± 158.7) yuan and the length of hospital stay was (25.5 ± 4.2) days, which were significantly lower than those in the control group (4 760.1 ± 245.6) yuan and (30.1 ± 5.6) days P <0.05). Satisfaction of patients in the treatment group was 96%, which was significantly higher than 78% in the control group (P <0.05). Conclusion Individualized nutrition supportive therapy for patients with tumor bone metastasis can significantly improve the prognosis of patients with nutritional index, significantly reduce the risk of malnutrition in patients, significantly reduce the cost of hospitalization and hospital stay, and can improve patient satisfaction and effective improvement of medical Suffering from the relationship, it is worth promoting clinical application.
