目的:探讨促性腺激素释放激素激动剂(GnRHa)治疗对特发性中枢性性早熟(ICPP)男童成年身高的影响。方法:对给予GnRHa进行治疗的123例ICPP男童进行随访研究,治疗前后分别测定患者血浆促黄体生成素(LH)、促卵泡生成素(FSH)水平,雌二醇(E2)水平,定期观测病人身高、体重、BMI指数、骨龄和结束治疗时的身高,对ICPP男童成年身高的主要影响因素进行Logistic分析。结果:经治疗后ICPP男童的LH、FSH和E2水平明显降低,趋向正常,与治疗前相比差异有统计学意义(P<0.05);随治疗时间的增加,男童的身高、体重和结束治疗时的身高均有明显升高,差异有统计学意义(P<0.05),而骨龄的增加速度和BMI指数升高不明显,与治疗前相比差异无统计学意义(P>0.05),表明经治疗后男童的生长发育受到抑制;ICPP男童成年身高与患者体重和BMI无明显相关(P>0.05),与结束治疗时的身高和靶身高显著相关(P<0.05);ICPP男童成年身高的主要影响因素为结束治疗时的身高和靶身高(P<0.05)。结论:GnRHa有助于改善ICPP男童的成年身高,主要影响因素取决于结束治疗时的身高和靶身高。
Objective: To investigate the effect of GnRHa treatment on the adult height of boys with idiopathic central precocious puberty (ICPP). Methods: A total of 123 ICPP boys treated with GnRHa were followed up. The levels of luteinizing hormone (LH), follicle stimulating hormone (FSH) and estradiol (E2) were measured before and after treatment. Patient height, weight, BMI, bone age and height at the end of treatment, the main influencing factors of adult height of ICPP boys were analyzed by Logistic analysis. Results: After treatment, the levels of LH, FSH and E2 in ICPP boys decreased significantly and tended to be normal (P <0.05). Compared with those before treatment, the boys’ height and weight and (P <0.05), while the increase of bone age and the increase of BMI index were not obvious, there was no significant difference compared with before treatment (P> 0.05) , Indicating that the growth of boys was inhibited after treatment. The height of adult ICPP boys was not significantly correlated with body weight and BMI (P> 0.05), and was significantly related to the height and target height at the end of treatment (P <0.05). ICPP The main influencing factor of adult boy height is height and target height at the end of treatment (P <0.05). CONCLUSIONS: GnRHa contributes to improving the adult height of ICPP boys, the main influencing factor being the height and target height at the end of treatment.
