目的分析2008-2014年徐州市城乡居民糖尿病死亡特征及变化趋势,为开展有针对性的干预措施提供依据。方法 2008-2014年所有徐州市户籍居民死亡资料来源于覆盖全市的居民死因登记报告系统。采用U检验、t检验、Spearman等级相关、年估计百分比变化(EAPC)对2008-2014年徐州市城乡居民糖尿病死亡数据进行分析。结果 2008-2014年徐州市居民年均糖尿病死亡率为8.84/10万,其中男性为8.35/10万,女性为9.36/10万,男性年均糖尿病死亡率低于女性,差异有统计学意义(P<0.05)。城区年均糖尿病死亡率为19.00/10万,其中城区男性为18.06/10万,女性为19.97/10万,除2012年城区男女性糖尿病死亡率差异有统计学意义(U=2.23,P<0.05)外,其余年份男女性差异均无统计学意义(P>0.05)。农村年均糖尿病死亡率为7.22/10万,其中男性为6.83/10万,女性为7.63/10万,2012年以前农村男女糖尿病死亡率差异均无统计学意义(P>0.05),2012年及以后,女性糖尿病死亡率高于男性,差异均有统计学意义(P<0.05)。城区男女性糖尿病死亡率均高于农村,差异均有统计学意义(P<0.05)。7年间,徐州市户籍居民糖尿病死亡率以每年7.02%的速度增长(P<0.05),其中,女性糖尿病死亡率以每年8.48%的速度上升,趋势有统计学意义(P<0.05),城区、农村女性糖尿病死亡率分别以每年6.54%、9.28%的速度上升,趋势有统计学意义(P<0.05),男性死亡率无明显差异。不论城区还是农村,7年间,各年龄段以及男性糖尿病死亡率变化均无统计学意义(P>0.05)。徐州市男性平均死亡年龄为(71.1±11.8)岁,女性为(72.4±12.3)岁,城市为(74.6±11.6)岁,农村为(70.6±12.0)岁,男性平均死亡年龄低于女性,城市高于农村,差异均有统计学意义(P<0.05)。城市男性、女性、农村女性居民因糖尿病死亡的平均年龄均呈逐年增长趋势(P<0.01)。结论近年来徐州市糖尿病死亡率呈上升趋势,应全面开展糖尿病的三级预防,并将女性、农村纳入重点干预对象。
Objective To analyze the characteristics and trends of death of diabetes mellitus among urban and rural residents in Xuzhou City during 2008-2014 and provide the basis for the targeted interventions. Methods All the deaths of registered permanent residents in Xuzhou from 2008 to 2014 are from the registration system of residents’ cause of death covering the whole city. The data of death from diabetes mellitus in urban and rural areas of Xuzhou during 2008-2014 were analyzed by U test, t test, Spearman rank correlation and annual estimated percentage change (EAPC). Results The average annual death rate of diabetes in Xuzhou city from 2008 to 2014 was 8.84 / 100000, of which 8.35 / 100000 for males and 9.36 / females for females. The average annual mortality rate of diabetes in males was lower than that in females (the difference was statistically significant ( P <0.05). The average annual death rate of diabetes mellitus in urban area was 19.00 / lakh, including 18.06 / lakh for urban area and 19.97 / lakh for female. There was significant difference in the mortality rate of male and female diabetes mellitus in urban area in 2012 (U = 2.23, P <0.05 ), There was no significant difference between male and female in other years (P> 0.05). The average annual death rate of diabetes in rural areas was 7.22 / lakh, of which 6.83 / lakh for males and 7.63 / lakh for females. There was no significant difference in mortality rates of diabetes between men and women in rural areas before 2012 (P> 0.05). In 2012 and Later, the mortality rate of diabetes in women than in men, the difference was statistically significant (P <0.05). Urban male and female diabetes mortality were higher than in rural areas, the difference was statistically significant (P <0.05). In the seven years, the death rate of diabetes in the permanent residents of Xuzhou increased by 7.02% per year (P <0.05). Among them, the death rate of diabetes in women increased by 8.48% per year with a trend of statistical significance (P <0.05) The death rate of diabetic women in rural areas increased by 6.54% and 9.28% per year, respectively. The trend was statistically significant (P <0.05). There was no significant difference in male mortality. In both urban and rural areas, there was no significant difference in mortality among all age groups and men in seven years (P> 0.05). The average age of death in male was (71.1 ± 11.8) years old in Xuzhou, (72.4 ± 12.3) years in female, (74.6 ± 11.6) in urban and (70.6 ± 12.0) years in rural. The average age of death in male was lower than that in female Higher than in rural areas, the difference was statistically significant (P <0.05). The average age of urban men, women and rural women dying from diabetes increased year by year (P <0.01). Conclusion In recent years, the mortality rate of diabetes in Xuzhou City is on the rise. Third-level prevention of diabetes should be carried out comprehensively and women and rural residents should be included in the key interventions.
