目的了解甲状腺功能低下(CH)患儿的早期治疗及治疗依从性对患儿智能发育的影响。方法选取邢台市2010-2014年经新生儿筛查确诊的甲状腺功能低下患儿46例为研究对象,选用Gesell发育量表评估患儿智能发育情况。结果出生后3个月后开始治疗患儿除语言外余各个能区DQ均落后于3个月内开始治疗患儿,差异有统计学意义(P<0.05);9例超3个月开始治疗患儿经规范治疗、康复锻炼半年后,适应性、大运动、精细运动及个人-社交4个能区DQ均有所提高,差异有统计学意义(P<0.05);完全配合治疗组患儿平均发育年龄与平均智测年龄相比,差异无统计学意义(P>0.05);不配合治疗组患儿各能区DQ均落后于完全配合治疗组,差异均有统计学意义(P<0.05)。结论患儿起始治疗时间及患儿的治疗依从性直接影响患儿治疗效果;及早规范替代治疗可以避免患儿智能发育低下的发生。
Objective To investigate the effect of early treatment and compliance of treatment on the mental development of children with hypothyroidism (CH). Methods Forty-six children with hypothyroidism confirmed by neonatal screening in Xingtai from 2010 to 2014 were selected as the research objects. The Gesell developmental scale was used to evaluate the intelligence development in children. Results After 3 months of birth, children with treatment of all kinds of energy except for the language beyond the start of DQ lag within 3 months after treatment, the difference was statistically significant (P <0.05); 9 cases started treatment beyond 3 months Children with standardized treatment, rehabilitation exercise six months later, adaptability, exercise, fine exercise and personal - social DQ 4 areas have increased, the difference was statistically significant (P <0.05); complete with the treatment group of children The mean developmental age was not significantly different from the mean intelligence age (P> 0.05). However, the DQ in different energy areas in the non-cooperative treatment group lagged behind that in the completely matched treatment group (P <0.05) ). Conclusion The initial treatment time in children and the treatment compliance in children directly affect the therapeutic effect in children. Early standardized alternative treatment can prevent the occurrence of mental retardation in children.
