目的 :探讨脑梗死患者血清NSE水平与脑损伤程度及垂体 -肾上腺 (PA)轴激素的相关性。方法 :采用放射免疫分析测定 5 5例脑梗死患者血清ACTH、Cor及NSE水平。结果 :小面积梗死组血清ACTH水平略高于正常对照组 ,无显著差异 (p >0 0 5 ) ,中及大面积组水平升高更为显著 (p <0 0 1,p <0 0 1) ;病理组之间 ,小面积与中面积组和中面积与大面积组两配对组间差异亦极显著 (p <0 0 1,p <0 0 1)。Cor水平则小面积组显著高于正常对照组 (p <0 0 5 ) ,中及大面积组水平升高更甚 (p <0 0 1,p <0 0 1) ;病理组间 ,小面积与中面积和中面积与大面积两两配对组间差异也极其显著 (p <0 0 1,p <0 0 1)。NSE水平小面积组显著高于正常对照组 (p <0 0 5 ) ,中及大面积组水平均极显著高于正常对照组 (p <0 0 1,p <0 0 1) ;病理组间 ,小面积与中面积和中面积与大面积两配对组间水平也具显著性 (p <0 0 5 ,p <0 0 5 )。以中面积组NSE与ACTH及Cor分别行直线相关分析 ,结果两者间均呈显著正相关 (r =0 6 2 ,r =0 6 6 ;p <0 0 1,p <0 0 1) )。 结论 :NSE水平的升高与脑梗死的病情关系密切 ;而两项PA轴激素的测定对PA轴内分泌调节功能的评估有重要价值
Objective: To investigate the correlation between the level of serum NSE and the degree of brain injury and the pituitary-adrenal (PAS) axis hormone in patients with cerebral infarction. Methods: Serum levels of ACTH, Cor and NSE in 55 patients with cerebral infarction were measured by radioimmunoassay. Results: Serum ACTH level in the infarct group was slightly higher than that in the normal control group (p> 0.05), and the level of ACTH in the medium and large area group was more significant (p <0.01, p <0.01 ). There were also significant differences between the two groups (P <0.01, p <0.01) between pathological groups, small area and medium area group, medium area and large area group. Cor level in the small area group was significantly higher than that in the normal control group (p <0 05), and in the middle and large area group was even higher (p <0.01, p <0.01) There was also a significant difference between pairwise paired groups with medium and medium area and large area (p <0.01, p <0.01). NSE level of small area group was significantly higher than that of normal control group (p <0 05), middle and large area group were significantly higher than the normal control group (p <0.01, p <0.01) (P <0 05, p <0 05) between the small area and the medium area and between the medium area and the large area. The linear correlation analysis between NSE, ACTH and Cor in middle-area group showed a significant positive correlation (r = 0 6 2, r = 0 6 6; p <0 0 1, p 0 01) . CONCLUSIONS: Elevated NSE is closely related to the severity of cerebral infarction. The determination of two PA axis hormones is of great value in the assessment of the endocrine-modulating function of the PA axis
