目的评价阴茎远段预置尿道分期手术治疗伴有阴茎背侧包皮组织缺乏重度尿道下裂的效果。方法采用阴茎远段预置尿道分期手术治疗23例伴有阴茎背侧包皮组织缺乏重度尿道下裂患儿,所有患儿均为初次行尿道成形手术。术后随访8~24个月。观察并分析手术治疗效果。结果 23例患儿中,无尿道狭窄、皮瓣坏死、尿道憩室、龟头裂开、阴茎下弯复发等,4例术后出现尿道瘘,6个月后行尿道瘘修补术治愈。结论伴有阴茎背侧包皮组织缺乏重度尿道下裂的患儿采用阴茎远段预置“尿道”分期手术,手术简单,成功率高,并发症少,手术效果好。
Objective To evaluate the effect of preoperative urethral staging of the penile distal meridian associated with the absence of severe hypospadias in the penile dorsal foreskin. Methods Twenty-three cases of children with severe hypospadias without dural circumcision were treated with distal urethral staging. All patients underwent urethroplasty for the first time. The patients were followed up for 8-24 months. Observation and analysis of surgical treatment. Results Of the 23 children, no urethral stricture, necrosis of the flap, urethral diverticulum, glans rupture and penile recurrence were found. Four cases had urethral fistula after operation and urethral fistula repair was performed after six months. Conclusions The pediatric patients with severe hypospadias with penile dorsal prepuce lack of preoperative “urethral” staging, the operation is simple, high success rate, fewer complications, and good surgical results.
