目的观察新生儿乙型肝炎(乙肝)疫苗初免及加强免疫应答与出生体重的关系。方法使用寿光市新生儿乙肝疫苗免疫持久性随访研究资料,选择2009年出生,完成了新生儿乙肝疫苗全程免疫和免后抗体检测,足月出生,出生体重记录清楚的儿童为观察对象。对乙肝疫苗初免和加强免疫检测结果进行统计分析。结果 968名新生儿乙肝疫苗全程免疫后抗-HBs保护应答率随出生体重增加而增高,差异有统计学意义(χ2=16.847,P=0.005);抗-HBs几何平均滴度(GMT)与出生体重存在正相关关系(rs=0.083,P=0.010)。免疫后5年,对抗-HBs<10 m IU/ml的儿童进行1剂次乙肝疫苗加强免疫,加强免疫后抗体水平与出生体重无相关性(rs=0.011,P=0.875)。结论出生体重是新生儿乙肝疫苗免疫应答的影响因素,出生体重与初免应答存在正相关关系,而与加强免疫则无相关性。
Objective To observe the relationship between neonatal hepatitis B (HB) vaccine initial immunization and enhancing immune response and birth weight. Methods According to the long-term follow-up study of immunization of newborns with hepatitis B vaccine in Shouguang City, we selected born in 2009 and completed the whole-body immunization and post-mortem antibody test of new-born hepatitis B vaccine. The full-term births and birth weight were recorded. The hepatitis B vaccine prime and boost immune test results for statistical analysis. Results The response rate of anti-HBs protection after 968 newborn hepatitis B vaccines was increased with birth weight gain (χ2 = 16.847, P = 0.005). The mean geometric mean titers of anti-HBs There was a positive correlation between body weight (rs = 0.083, P = 0.010). Five years after immunization, children with anti-HBs <10 m IU / ml were boosted with one dose of hepatitis B vaccine. There was no correlation between antibody levels and birth weight after immunization (rs = 0.011, P = 0.875). Conclusions Birth weight is the influencing factor of neonatal hepatitis B vaccine immune response. There is a positive correlation between birth weight and initial immune response, but no correlation with enhanced immunity.
