目的探讨螺旋CT与胃肠道造影对原发性小肠肿瘤的诊断价值。方法选择唐山市中医医院2011年10月~2014年10月收治的经病理诊断确诊的原发性小肠肿瘤患者46例,均经手术治疗。术前均经螺旋CT与胃肠道造影检查,比较两种方法对小肠肿瘤的检出率及诊断的准确率。结果胃肠造影对小肠疾病的诊断结果:46例患者中,7例向腔外生长的淋巴瘤、9例腔外型平滑肌肉瘤、4例腔外型间质瘤及1例息肉患者未检出,检出率为54.3%(25/46);检出的25例患者中,有1例间质瘤、1例平滑肌肉瘤误诊为腺癌。胃肠造影诊断小肠肿瘤的准确性为50.0%(23/46)。螺旋CT对小肠疾病的诊断结果:46例患者中,有1例间质瘤、1例息肉患者未检出,检出率为95.7%(44/46);检出的44例患者螺旋CT诊断结果与病理诊断结果均相符,诊断准确率为95.7%(44/46)。两种方法对小肠肿瘤检出率、诊断准确度比较,差异均有统计学意义(均P<0.05)。结论螺旋CT对原发性小肠肿瘤的检出率和诊断准确度均优于胃肠道造影,且操作简单,患者易接受,值得临床推广应用。
Objective To investigate the diagnostic value of spiral CT and gastrointestinal angiography in primary small intestinal tumors. Methods Forty-six patients with pathologically diagnosed primary small bowel cancer admitted to Tangshan Chinese Medicine Hospital from October 2011 to October 2014 were enrolled in this study. All of them were surgically treated. Preoperative spiral CT and gastrointestinal imaging were compared to compare the detection rate of two methods of small bowel tumors and the diagnostic accuracy. Results Gastrointestinal angiography was used to diagnose small bowel disease. Of the 46 patients, 7 were extracavitary lymphomas, 9 were extracavitary leiomyosarcomas, 4 were extracapsular extracapsular tumors, and 1 was not detected , The detection rate was 54.3% (25/46). Of the 25 patients detected, 1 had stromal tumors and 1 had leiomyosarcoma misdiagnosed as adenocarcinoma. Gastrointestinal angiography in the diagnosis of small bowel tumors was 50.0% (23/46). Spiral CT diagnosis of small bowel disease: 46 cases of patients, 1 case of stromal tumors, 1 case of polyps were not detected, the detection rate was 95.7% (44/46); detected in 44 patients with spiral CT diagnosis The results were consistent with the pathological diagnosis, diagnostic accuracy was 95.7% (44/46). The two methods for the detection rate of small intestine tumors, diagnostic accuracy, the differences were statistically significant (P <0.05). Conclusion The detection rate and diagnostic accuracy of spiral CT in primary small intestinal tumors are superior to those in gastrointestinal tract. The operation is simple and the patients are easy to accept. It is worthy of clinical application.
