目的了解宁夏银川、吴忠两市中小学生脂代谢相关指标的异常情况,为预防儿童相关慢性疾病的发生提供基础数据。方法在银川市兴庆区、金凤区、西夏区和吴忠市分层随机整群抽取小学、初中和高中在校学生1 786名,对抽取的中小学生进行身高、体重的测量,采集空腹静脉血,测定总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)。结果银川、吴忠两市中小学生脂代谢相关指标TC,TG,LDL-C,HDL-C的异常检出率分别为1.5%,14.5%,0.4%,33.7%,男生LDL-C异常检出率(0.8%)高于女生(0.1%),差异有统计学意义(χ~2=4.825,P=0.028)。脂代谢相关指标年龄分布,TC,LDL-C异常集中在9~11岁,而TG,HDL-C异常检出率随年龄增长呈上升趋势;脂代谢水平比较,TC,LDL-C水平女生高于男生(t值分别为-2.602,-2.607,P值均<0.05),TC,LDL-C,HDL-C随年龄的增长呈下降的趋势,TG则随年龄的增长呈上升趋势,4项指标在年龄间差异均有统计学意义(F值分别为13.541,13.813,18.738,20.327,P值均<0.01)。回、汉族中小学生脂代谢异常检出率差异均无统计学意义;汉族学生TC水平高于回族学生,差异有统计学意义(t=2.990,P=0.003)。TC,TG,LDL-C异常检出率在不同体型学生间差异均有统计学意义,肥胖组和超重组学生的TC,TG,LDL-C水平均高于正常组学生(P值均<0.05),且均随BMI增加而逐渐升高。结论银川、吴忠两市中小学生脂代谢相关指标的异常检出率不容乐观,超重和肥胖是中小学生脂代谢异常的影响因素之一。需重视中小学生肥胖的预防,对肥胖超重的学生进行脂代谢的相关检查和必要干预。
Objective To understand the abnormalities of lipid metabolism related indicators of primary and secondary school students in Yinchuan and Wuzhong of Ningxia and provide basic data for preventing the occurrence of chronic diseases related to children. Methods A total of 1 786 students were enrolled in primary schools, middle schools and high schools in Xingqing district, Jinfeng district, Xixia district and Wuzhong city of Yinchuan city. The height and weight of primary and middle school students were measured. Results The abnormal detection rates of TC, TG, LDL-C and HDL-C in primary and secondary school students in Yinchuan and Wuzhong were 1.5%, 14.5%, 0.4% and 33.7% (0.8%) than girls (0.1%), the difference was statistically significant (χ ~ 2 = 4.825, P = 0.028). The TC level of Han students was higher than that of Hui students (t = 2.990, P = 0.003). The detection rates of TC, TG and LDL-C were significantly different in different body size students. The levels of TC, TG and LDL-C in obese and overweight students were significantly higher than those in normal students (P <0.05) ), And gradually increased with the increase of BMI. Conclusion The abnormal detection rate of lipid metabolism related indicators in primary and secondary school students in Yinchuan and Wuzhong is not optimistic. Overweight and obesity are one of the influencing factors of abnormal lipid metabolism in primary and secondary school students. Should pay attention to the prevention of obesity in primary and secondary students, fat overweight students lipid metabolism related checks and the necessary intervention.
