目的探讨急性一氧化碳中毒(ACOP)患者应用磷酸肌酸钠对其心肌损害的改善效果。方法选取2015年4月至2016年3月收治的100例ACOP患者,依据随机数字表法分为两组,各50例。对照组给予常规综合治疗,观察组在此基础上给予磷酸肌酸钠治疗。比较两组治疗前后N末端B型利钠肽原(NT-pro BNP)、肌红蛋白(MYO)、肌钙蛋白I(c Tn I)及肌酸激酶同工酶(CK-MB)水平,治疗前后QT间期离散度(QTd)、最大QT间期(QTmax)、最小QT间期(QTmin)、HR及左室射血分数(LVEF),治疗期间不良反应情况。结果两组治疗后NTpro BNP、MYO、c Tn I及CK-MB水平与治疗前比较,均显著降低(P均<0.01),且治疗后观察组各指标水平优于对照组(P均<0.01);两组治疗后QTd、QTmin、HR及LVEF水平均显著优于治疗前(P均<0.01),且治疗后观察组优于对照组(P均<0.01);但两组QTmax治疗前后无明显变化(P均>0.05)。结论 ACOP患者应用磷酸肌酸钠治疗能有效改善心肌损伤标志物水平及心电图与超声心动图状况,保护心肌组织,疗效切实,安全可靠。
Objective To investigate the effect of sodium creatine phosphate on myocardial damage in patients with acute carbon monoxide poisoning (ACOP). Methods A total of 100 patients with ACOP who were admitted from April 2015 to March 2016 were divided into two groups according to the random number table (50 cases). The control group was given conventional comprehensive treatment, and the observation group was treated with sodium creatine phosphate on the basis of this. The levels of N-terminal pro-brain natriuretic peptide (NT-pro BNP), myoglobin (MYO), troponin I (cTn I) and creatine kinase MB (CK-MB) QTd, QTmax, QTmin, HR and LVEF before and after treatment, adverse reactions during treatment. Results The levels of NTpro BNP, MYO, cTn I and CK-MB in the two groups were significantly lower than those before treatment (all P <0.01), and the indexes in the observation group were better than those in the control group after treatment ). The levels of QTd, QTmin, HR and LVEF in both groups were significantly better than those before treatment (all P <0.01), and after treatment, the observation group was better than the control group (all P <0.01) Significant changes (P> 0.05). Conclusion ACOP patients treated with sodium creatine phosphate can effectively improve the level of myocardial injury markers and ECG and echocardiography, protect myocardial tissue, effective, safe and reliable.
