[目的]探讨超声结合螺旋CT在结肠癌诊断中的价值。[方法]选取40例进行超声及螺旋CT检查并行结肠癌根治术的患者,对影像检查结果与术后病理诊断结果进行比较。[结果]螺旋CT与超声比较,病理符合率差异无统计学意义(P>0.05);超声+螺旋CT与超声或者螺旋CT比较,病理符合率差异均有统计学意义(P<0.05)。超声诊断出29例结肠“假肾征”样肿块,2例索条状肿块,5例不规则形肿块,3例有不规则坏死液化,1例弧形强回声。肿块内有彩色血液。漏误诊9例。螺旋CT诊断出结肠软组织肿块(包括8例坏死,5例钙化)23例,肠壁增厚伴肠腔狭窄12例。漏误诊6例。术前超声诊断淋巴结转移7例,与手术病理诊断(23例)比较,差异有统计学意义(P<0.05);螺旋CT诊断淋巴结转移13例,与手术病理诊断(23例)比较,差异有统计学意义(P<0.05);超声+螺旋CT诊断淋巴结转移19例,与手术病理诊断(23例)比较,差异无统计学意义(P>0.05)。[结论]超声结合螺旋CT可提高结肠癌术前诊断准确率。
[Objective] To investigate the value of ultrasound combined with spiral CT in the diagnosis of colon cancer. [Method] Forty patients with concurrent colonoscopies undergoing ultrasound and spiral CT were enrolled in this study. The results of imaging and postoperative pathological diagnosis were compared. [Result] There was no significant difference in pathological coincidence between spiral CT and ultrasound (P> 0.05). There was significant difference between pathological coincidence rate of ultrasound + spiral CT and ultrasound or spiral CT (P <0.05). Ultrasound diagnosis of 29 cases of colon “false kidney sign” like mass, 2 cases of cord-like mass, 5 cases of irregular shape mass, 3 cases of irregular necrosis liquefaction, 1 case of strong echo. There are colored blood within the tumor. Miss misdiagnosis in 9 cases. Spiral CT diagnosis of soft tissue mass in the colon (including 8 cases of necrosis, 5 cases of calcification) in 23 cases, intestinal wall thickening with intestinal stenosis in 12 cases. Miss misdiagnosed 6 cases. Lymph node metastasis was diagnosed by ultrasound before operation in 7 cases, which was significantly different from that in 23 cases (P <0.05). There were 13 cases with lymph node metastasis by spiral CT and 23 cases with pathological diagnosis Statistical significance (P <0.05). Ultrasound + spiral CT in 19 cases of lymph node metastasis, with the surgical pathological diagnosis (23 cases), the difference was not statistically significant (P> 0.05). [Conclusion] Ultrasound combined with spiral CT can improve the accuracy of preoperative diagnosis of colon cancer.
