目的探讨右美托咪定对脓毒症患者围术期血浆中高迁移率族蛋白B1(HMGB1)的影响。方法 60例脓毒症患者,随机分为生理盐水组(NS组)和右美托咪定组(DEX组),每组30例。分别于麻醉诱导前(T0)、手术开始时(T1)、手术结束时(T2)及术后24 h(T3)采集静脉血,应用酶联免疫吸附试验(ELISA)测定血浆HMGB1的浓度。结果在T0、T1、T2和T3时间点,两组患者血浆中HMGB1水平均随时间升高,T3时间点达到高峰。DEX组患者在T2和T3时间点血浆中HMGB1水平低于NS组(P<0.05)。结论右美托咪定可明显降低脓毒症患者围术期血浆HMGB1水平,改善患者预后,可在临床推广。
Objective To investigate the effect of dexmedetomidine on high mobility group box 1 protein (HMGB1) in patients with sepsis during perioperative period. Methods Sixty sepsis patients were randomly divided into normal saline group (NS group) and dexmedetomidine group (DEX group), 30 cases in each group. Venous blood samples were collected before induction of anesthesia (T0), at the start of surgery (T1), at the end of surgery (T2) and 24 h after operation (T3) respectively. The concentrations of plasma HMGB1 were measured by enzyme-linked immunosorbent assay (ELISA) Results At the time points of T0, T1, T2 and T3, the plasma levels of HMGB1 in both groups increased with time and reached the peak at T3. The plasma levels of HMGB1 in patients with DEX at T2 and T3 were lower than those in patients with NS (P <0.05). Conclusion Dexmedetomidine can significantly reduce perioperative plasma HMGB1 levels in patients with sepsis and improve prognosis in patients with clinical promotion.
