目的探讨高频彩色多普勒超声在乳腺癌筛查中的应用价值。方法乳腺癌筛查发现并经病理证实的乳腺癌48例,回顾性总结高频彩色多普勒超声检查的特点及与钼靶X线摄影的互补作用。结果超声诊断乳腺癌43例(89.6%),声像特点为形态不规则或分叶状低回声肿块、纵横比>1、边界不清有毛刺或蟹足、可见微小钙化;假阴性5例钼靶摄影均见微小钙化灶,局部结构稍紊乱,也未见明确肿块影,术后病理示髓样癌。乳腺钼靶X线摄影诊断乳腺癌41例(85.42%),7例假阴性均未见微小钙化,高频超声检查均可见低回声肿块声像(直径范围0.4~1.5cm),部分边界不清,后方伴衰减。两种方法联合应用乳腺癌检出率为100%。结论高频彩色多普勒超声检查便捷、无创、重复性好,对乳腺癌诊断敏感性高,与钼靶X线摄影结合可明显提高检出率和准确率。
Objective To investigate the value of high-frequency color Doppler ultrasound in breast cancer screening. Methods Forty - eight cases of breast cancer were screened by breast cancer and confirmed by pathology. The characteristics of high - frequency color Doppler ultrasonography and its complementarity with mammography were retrospectively reviewed. Results Ultrasound diagnosis of breast cancer in 43 cases (89.6%), the image features of irregular or lobulated hypoechoic mass, aspect ratio> 1, the border is unclear with burr or crab foot, visible micro-calcification; false negative 5 cases of molybdenum Target photography were seen in small calcification, the local structure of a little disorder, no clear mass shadow, postoperative pathology showed medullary carcinoma. There were 41 cases (85.42%) diagnosed with breast cancer by mammography and 7 cases with false-negative findings. High-frequency ultrasonography showed hypoechoic mass (0.4-1.5 cm in diameter), unclear border, Rear with attenuation. Breast cancer combined detection of the two methods was 100%. Conclusion High-frequency color Doppler ultrasonography is convenient, noninvasive, and reproducible. It is highly sensitive to the diagnosis of breast cancer and can be combined with mammography to significantly improve the detection rate and accuracy.
