目的分析研究结核分枝杆菌T细胞斑点实验(T-SPOT.TB)诊断肺结核的临床应用价值。方法 125例疑似肺结核患者,分别采用T-SPOT.TB、结核分枝杆菌抗体(TB-AB)和痰结核分枝杆菌DNA(TB-DNA)3种不同方法对其实施检测,判断确诊肺结核患者,并对不同方法诊断肺结核的敏感性、特异性进行比较。结果 125例患者中,符合肺结核临床诊断标准并确诊的有101例,其中T-SPOT.TB、TB-AB、TB-DNA诊断肺结核的敏感性分别为83.2%、52.5%、43.6%,T-SPOT.TB法敏感性高于TB-AB和TB-DNA法(P<0.05),其诊断肺结核的特异性分别为79.2%、62.5%、83.3%,T-SPOT.TB和TB-DNA法特异性高于TB-AB(P<0.05)。结论 T-SPOT.TB诊断肺结核的敏感性和特异性都比较高,是诊断肺结核的有效手段。
Objective To analyze the clinical value of T-SPOT.TB in the diagnosis of pulmonary tuberculosis. Methods Totally 125 cases of suspected pulmonary tuberculosis patients were tested by T-SPOT.TB, TB-AB and TB-DNA, , And compared different methods to diagnose the sensitivity and specificity of pulmonary tuberculosis. Results Of the 125 patients, 101 were diagnosed and clinically diagnosed as pulmonary tuberculosis. The sensitivity of T-SPOT.TB, TB-AB and TB-DNA in diagnosis of pulmonary tuberculosis were 83.2%, 52.5% and 43.6%, respectively. The T- The sensitivity of SPOT.TB method was higher than that of TB-AB and TB-DNA method (P <0.05). The specificity of SPOT.TB method was 79.2%, 62.5% and 83.3% respectively. The specificity of T-SPOT.TB and TB-DNA Sex is higher than TB-AB (P <0.05). Conclusion The sensitivity and specificity of T-SPOT.TB in the diagnosis of pulmonary tuberculosis are relatively high, which is an effective method to diagnose pulmonary tuberculosis.
