目的了解吸入外源性一氧化氮(NO)对快速进入高原现场幼猪(其情形近似于急性高原病患者)氧代谢动力学的影响。方法利用右心漂浮导管法快速进入高原现场幼猪进入高原后及在高原吸入外源性NO后氧代谢动力学的变化情况进行比较观察。结果快速进入高原现场幼猪吸入NO治疗前与平原比较肺动脉平均压(mPAP)、肺血管阻力(PVR)、肺内分流(Qs/Qt)显著升高,心输出量(CO)、氧输送(DO2)、氧消耗(VO2)、动脉血氧饱和度(SaO2)显著降低;吸入NO后,快速进入高原现场幼猪mPAP、PVR、Qs/Qt显著降低,CO、DO2、VO2、SaO2显著升高;以上各指标均P<0.01;氧摄取率(O2Ext)无明显变化。结论吸入外源性NO能显著降低快速进入高原幼猪的PVR,并通过增加CO而提高组织的DO2、VO2和SaO2,从而显著改善幼猪机体组织因缺氧引起的氧化谢障碍,对急性缺氧有着积极的治疗作用。
Objective To investigate the effects of inhaled exogenous nitric oxide (NO) on the oxygen metabolism kinetics of piglets entering the plateau rapidly (in the case of acute altitude sickness). Methods Right heart floating catheter method quickly entered the plateau piglets into the plateau after high altitude inhalation of exogenous NO changes in oxygen metabolism kinetics were compared. Results Compared with the plain before treatment, NO intake of NO in young pigs entering the plateau significantly increased pulmonary vascular mean pressure (mPAP), pulmonary vascular resistance (PVR), pulmonary Qs / Qt, cardiac output (CO), oxygen delivery DO2, VO2 and SaO2 decreased significantly. After inhaling NO, the mPAP, PVR and Qs / Qt of piglets rapidly entering the plateau decreased significantly and CO, DO2, VO2 and SaO2 increased significantly ; All above indexes were all P <0.01; there was no obvious change in oxygen uptake rate (O2Ext). Conclusion Inhalation of exogenous NO can significantly reduce the PVR of piglets entering the plateau quickly and increase the tissue DO2, VO2 and SaO2 by increasing the CO, thereby significantly improving the oxidative and metabolic disorders of piglets due to hypoxia, Oxygen has a positive therapeutic effect.
