目的探讨瘦素受体(LEPR)基因多态性与未绝经女性近5年体质指数(BMI,)变化之间的关系。方法采用横断面研究,于2015年3月~6月收集四川省人民医院体检中心和成都双流区妇保院体检中心未绝经健康女性463人,收集其一般人口学特征、人口测量指标、日均能量摄入等信息,以5年前BMI与现在的BMI变化情况将研究对象分为低风险组(BMI5year<24 kg/m~2→BMI_(present)<24 kg/m~2&BMI_5year≥24 kg/m~2→BMI_(present)<24 kg/m~2)、增重至超重组(BMI_5year<24kg/m~2→BMI_(present)≥24kg/m)、持续超重/肥胖组(BMI_5year≥24 kg/m2→BMI_(present)≥24kg/m~2)。采用飞行时间质谱检测LEPRrs1137100,LEPRrs1137101基因型。采用多分类Logistic回归,估计LEPR基因多态性与未绝经健康女性人群近5年BMI变化的关联值(OR)。结果 LEPR rs1137100和LEPR rs1137101突变等位基因的携带率分别为83.3%和89.0%;调整重要的混杂因素后,携带LEPRrs1137100AA+AG或LEPRrs1137101AA+AG可增加发生持续超重/肥胖的风险(OR=1.67,95%CI:1.02-2.73;OR=1.76,95%CI:1.01-3.07)。结论LEPRrs1137100和LEPRrs1137101基因多态性可能与未绝经女性持续超重/肥胖有关。
Objective To investigate the relationship between polymorphism of leptin receptor (LEPR) gene and body mass index (BMI) in postmenopausal women over the past five years. Methods A cross-sectional study was conducted. From March to June 2015, 463 non-menopausal healthy women from the Physical Examination Center of Sichuan Provincial People’s Hospital and from the Chengdu Maternity Medical Examination Center were collected and their demographic characteristics, Energy intake and other information, the BMI5year <24 kg / m ~ 2 → present <24 kg / m ~ 2 & BMI_5year≥24 kg / (BMI_5year <24kg / m 2 → BMI present24kg / m), continuous overweight / obesity group (BMI_5year≥24 kg / m2 → BMI present 24kg / m ~ 2). The LEPRrs1137100 and LEPRrs1137101 genotypes were detected by time of flight mass spectrometry. Multi-classification Logistic regression was used to estimate the association of LEPR polymorphism with BMI in the 5-year healthy female population. Results The carrying rates of LEPR rs1137100 and LEPR rs1137101 were 83.3% and 89.0% respectively. After adjusting for important confounding factors, LEPR rs1137100AA + AG or LEPRrs1137101AA + AG could increase the risk of persistent overweight / obesity (OR = 1.67, 95% CI: 1.02-2.73; OR = 1.76, 95% CI: 1.01-3.07). Conclusion The polymorphisms of LEPRrs1137100 and LEPRrs1137101 may be related to persistent overweight / obesity in non-menopausal women.
