目的观察多廿烷醇治疗老年高脂血症的有效性及安全性。方法选择血脂异常老年患者34例,以多廿烷醇20 mg·d-1口服,疗程24周。用药前后检测患者空腹血糖,血脂变化,肝、肾功能等,观察多廿烷醇调脂疗效及安全性;行肱踝脉搏波传导速度(ba PWV)检查,了解其对大动脉弹性的作用;行SF-36量表检测,以评价药物对患者生活质量的影响。结果多廿烷醇治疗24周,血清总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)水平下降幅度分别为17.5%、23.6%和19.3%(P<0.05或P<0.01)。多廿烷醇治疗后,SF36值显著升高[125.32±13.93 vs.115.28±15.51(治疗前),P<0.05];用药治疗后除生理职能外,活力、生理机能、身体疼痛、总体健康、社会功能、情感职能和精神健康均有显著改善(P<0.05)。多廿烷醇治疗后ba PWV下降显著[1 527.87±232.09 vs.1 603.97±215.55(治疗前),P<0.01]。多廿烷醇治疗后血压、心率、肝功能、肾功能、肌酸激酶等均未发现显著变化(P>0.05);未有患者出现心肌激酶或丙氨酸转氨酶升高等不良事件。结论多廿烷醇治疗老年血脂异常患者,其调脂作用显著,可改善患者生活质量及大动脉弹性,安全性良好。
Objective To observe the efficacy and safety of policosanol in the treatment of senile hyperlipidemia. Methods 34 cases of elderly patients with dyslipidemia, policosanol 20 mg · d-1 orally for 24 weeks. Before and after treatment, the fasting blood glucose, blood lipid changes, liver and kidney function were measured to observe the efficacy and safety of policosanol lipid regulation. The brachial-ankle pulse wave velocity (ba PWV) was used to examine the effect on the elasticity of the aorta. SF-36 scale test to evaluate the impact of drugs on the quality of life of patients. Results After 24 weeks policosanol treatment, the levels of serum total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) and triglyceride (TG) decreased by 17.5%, 23.6% and 19.3% Or P <0.01). In addition to physiological functions, the activity, physiological function, body ache, general health, Social function, emotional function and mental health were significantly improved (P <0.05). Ba PWV decreased significantly after policosanol treatment [1 527.87 ± 232.09 vs.1 603.97 ± 215.55 (before treatment), P <0.01]. No significant changes were found in blood pressure, heart rate, liver function, renal function, creatine kinase after policosanol treatment (P> 0.05). No patients had any adverse events such as elevated myocardial kinase or alanine aminotransferase. Conclusion Policosanol treatment of elderly patients with dyslipidemia significant lipid-lowering effect, can improve the quality of life of patients and aortic elasticity, good safety.
