目的使用青岛市糖尿病危险评分表构建用于代谢综合征(MS)发生风险的预测工具,为代谢综合征的早期筛查方法、干预措施的制定提供科学依据。方法随机抽取青岛地区7 500名35~75岁常住居民作为研究对象进行调查,以青岛糖尿病危险评分值为自变量,以是否患有代谢综合征为结局变量绘制ROC曲线,计算灵敏度和特异度,并根据最大约登指数判断最佳切点。结果青岛市糖尿病危险评分筛查代谢综合征的最佳切点为14.50,曲线下面积(AUC)为0.77(95%CI0.75~0.78),灵敏度为73.67%,特异度为68.42%。其中男性的最佳切点为14.50,AUC为0.76(95%CI0.74~0.78),灵敏度为72.13%,特异度为69.28%;女性的最佳切点为15.50,AUC为0.77(95%CI0.75~0.78),灵敏度为74.68%,特异度为68.24%。青岛糖尿病危险评分表筛查代谢综合征的AUC明显高于腰高比。结论青岛市糖尿病危险评分筛查代谢综合征的准确性较高,是开展社区筛查和健康促进的最佳模型。
Objective To establish a prediction tool for the risk of metabolic syndrome (MS) using the Qingdao Diabetes Risk Score Scale, and provide a scientific basis for the early screening of metabolic syndrome and the formulation of interventions. Methods A random sample of 7 500 residents ranging in age from 35 to 75 in Qingdao was selected as the research object. The Qingdao diabetes risk score was taken as the independent variable, and the metabolic syndrome was taken as the outcome variable to draw the ROC curve to calculate the sensitivity and specificity. And according to the maximum Youden index to determine the best cut point. Results The best cut point of screening for metabolic syndrome in Qingdao was 14.50. The area under the curve (AUC) was 0.77 (95% CI 0.75 to 0.78), the sensitivity was 73.67% and the specificity was 68.42%. Among them, the best cut point for men was 14.50, the AUC was 0.76 (95% CI 0.74-0.78), the sensitivity was 72.13% and the specificity was 69.28%. The best cut point for women was 15.50 and the AUC was 0.77 (95% CI0 .75 ~ 0.78), the sensitivity was 74.68% and the specificity was 68.24%. Qingdao diabetes risk score for screening metabolic syndrome AUC was significantly higher than the waist-height ratio. Conclusion The screening of metabolic syndrome in Qingdao with high risk of diabetes is the best model to carry out community screening and health promotion.
