目的探讨戴明循环(PDCA循环)对小儿静脉留置针固定效果的影响。方法选取2013年1月至2014年12月沈阳市儿童医院收治的128例住院患儿作为研究对象,按随机数字表法将其分为对照组与观察组,各64例。对照组患儿行常规静脉留置针固定,观察组按照PDCA循环对护理人员进行管理,然后对患儿进行静脉留置针固定,比较两组患儿静脉炎、穿刺部位皮炎、意外脱落、外渗、敷料污染发生率及静脉留置针留置时间、患儿家属对护理工作的满意情况。结果观察组患儿静脉炎、穿刺部位皮炎、意外脱落、外渗及敷料污染发生率均明显低于对照组,差异均有统计学意义(均P<0.05);观察组患儿静脉留置针留置时间为(100±14)h,明显长于对照组的(82±10)h,差异有统计学意义(P<0.05);观察组家属对护理人员静脉穿刺操作能力、服务态度、沟通及安抚能力满意度均明显高于对照组,差异均有统计学意义(均P<0.05)。结论 PDCA循环在小儿静脉留置针管理中应用效果明显,能有效降低并发症发生率,延长静脉留置针留置时间,有利于提升患儿家属护理满意度。
Objective To investigate the effect of Deming cycle (PDCA cycle) on the fixation of infantile venous indwelling needle. Methods A total of 128 hospitalized children admitted to Shenyang Children’s Hospital from January 2013 to December 2014 were selected as study subjects and divided into control group and observation group according to random number table method, with 64 cases in each group. Control group of children underwent routine intravenous indwelling needle fixation, the observation group in accordance with the PDCA cycle for nursing staff management, and then the children were intravenous catheter fixation, phlebitis, puncture site dermatitis, accidental shedding, extravasation, Dressings pollution incidence and indwelling needle indwelling time, family members of nursing satisfaction. Results The incidence of phlebitis, puncture site dermatitis, accidental exfoliation, extravasation and dressing contamination in the observation group were significantly lower than those in the control group (all P <0.05). In the observation group, the indwelling needle The time was (100 ± 14) h, significantly longer than that of the control group (82 ± 10) h, the difference was statistically significant (P <0.05). The relatives of the observation group had significant differences in the venous puncture ability, service attitude, communication and appeasement Satisfaction were significantly higher than the control group, the difference was statistically significant (P <0.05). Conclusions PDCA circulation is effective in the management of infantile venous indwelling needle. It can effectively reduce the incidence of complications and prolong the indwelling time of venous indwelling needle, which is beneficial to improve the satisfaction of family members in children.
