遥测心电监护临床应用体会太原市中心医院（０３０００９）张丽丽，薛定荣，薛伟珍，张震芳太原市商业职工医院杨芝兰我们对３６７例拟诊为心血管疾病的患者，进行连续心电监测，本文监测出各种心律失常进行分类研究，现报告如下。临床资料男２０９例，女１５８例。年龄１６～８１岁，平均５１．７岁。其中冠心病（ＣＡＤ）合并急件心肌梗塞（ＡＭＩ）４２例；ＣＡＤ２０１例；晕厥或心悸待诊５９例；仅表现为室性早博６５例。所有病例均采用同一日产光电８４０３ｋ４／８人遥测心电监护仪连续心电监测，时间为２～３３６小时。结果一ＣＡＤ合并ＡＭＩ组：４２例中正常心律９例；其余３３例心津失常类型见附表。二、ＣＡＤ不伴有ＡＭＩ组：２０１例检出ＳＴ段改变１８１例，检出率９０．０５％（ＳＴ段判定参照ＣｏｈｎＰＦ标准［１］。其中合并心绞痛发作５７例，无症状性心肌缺血（ＳＭＩ）１１４例，ＳＴ抬高（＞２ｍｍ）１０例。ＳＴ段下移者多伴有心率增快，ＳＴ段抬高含多伴有心率减慢，ＳＭＩ在活动与休息时出现ＳＴ段变化的频度相似。三、晕厥与心悸组：５９例常规ＥＣＧ正常，２４小时内发现心律失常３３例。２３例晕厥者有７例为心源性晕厥；３６例心悸者检出２６例心律失常。四、单纯室早组
