目的:探讨脉冲染料激光(pulsed dye laser,PDL)不同治疗周期对兔耳创面上皮细胞向间质细胞转化的影响及该现象对瘢痕增生的作用。方法:新西兰大耳白兔12只建立兔耳腹侧增生性瘢痕模型,应用595nm Vbeam脉冲染料激光分别采用一周、二周、三周的治疗周期进行瘢痕的激光干预,在治疗前及治疗2、4、6、8周测量瘢痕厚度,治疗结束后取材行组织形态学观察,免疫组织化学方法测定各组上皮间充质转化(Epithelial-mesenchymal Transformation,EMT)正相关蛋白及负相关蛋白的表达情况。结果:兔耳各组模型均有瘢痕增生,EMT表面标记物表达阳性;PDL治疗结束后,一周治疗组及二周治疗组均表现出TGF-β1、Vim表达下调,Ecad表达上调,产生明显的瘢痕抑制效应,与对照组相比有显著差异;三周治疗组与对照组、一周治疗组和二周治疗组相比无明显差异。结论:PDL可能通过干预EMT的表达产生瘢痕抑制作用;治疗周期为两周较为适宜。
OBJECTIVE: To investigate the effect of pulsed dye laser (PDL) treatment on the transformation of epithelial cells into stromal cells in rabbit ear wounds and the effect of this treatment on scar hyperplasia. Methods: Twelve new Zealand white rabbits were used to establish the ventral hypertrophic scars model of rabbits. The rabbits were treated with 595nm Vbeam pulsed laser for one, two and three weeks respectively. Before and after treatment, The thickness of scars were measured at 4, 6 and 8 weeks. After the treatment, the histological changes of the samples were observed. The expressions of the positive and negative correlations of Epithelial-mesenchymal transformation (EMT) in each group were determined by immunohistochemistry . Results: All the rabbit ear models had scar hyperplasia and EMT surface marker positive expression. After PDL treatment, the expression of Vim and TGF-β1 were down-regulated and the expression of Ecad was up-regulated in the one-week and two-week treatment groups There was a significant difference between the three weeks treatment group and the control group, one week treatment group and two weeks treatment group. Conclusion: PDL may inhibit scarring by interfering with the expression of EMT. The treatment cycle is suitable for two weeks.
