目的 研究反映窒息新生儿肾损伤指标的敏感性。方法 测定48例窒息新生儿(重度25例,轻度23例)血清尿素氮、肌酐、尿酸与尿NAG、GGT、β2-MG和尿常规的变化。结果 重度窒息组各项指标的异常发生率高于轻度窒息组(P<005)。重度、轻度窒息组内尿NAG、GGT、β2-MG的异常发生率(833%和50%、833%和50%、833%和70%)高于血清BUN、Cr、Ua(333%和15%、5714%和40%、50%和333%)。结论 窒息新生儿普遍存在肾功能损害,尿酶、β2-MG和尿常规能及早反映肾功能损害,可作为监测窒息后损伤早期敏感指标。
Objective To study the sensitivity of neonatal renal injury indicators reflecting asphyxia. Methods The changes of serum urea nitrogen, creatinine, uric acid and urine NAG, GGT, β2-MG and urinary routine in 48 neonates with asphyxia (25 severe and 23 mild) were determined. Results The abnormal incidence of various indexes of severe asphyxia group was higher than that of mild asphyxia group (P <005). The abnormal incidence of urinary NAG, GGT and β2-MG in severe and mild asphyxia group was higher than that in serum BUN, Cr and Ua (333% and 50%, 833% and 50%, 833% and 70% 15%, 5714% and 40%, 50% and 333%). Conclusion Asphyxial newborns generally have renal dysfunction. Urease, β2-MG and urinary routine can reflect renal dysfunction as early as possible, which can be used as a sensitive index to monitor early post-asphyxial injury.
