目的:探讨胃增强CT与超声双重造影诊断胃间质瘤的临床价值。方法:收取2010年1月至2016年1月间于我院就诊的胃间质瘤患者43例作为研究对象,对其胃增强CT与超声双重造影检查结果进行回顾性分析,总结两种方法的诊断准确率。结果:将43例胃间质瘤患者分为高危型17例及低危型26例,分别纳入高危组及低危组。高危组肿瘤大小>5 cm患者例数明显多于低危组,有液化坏死现象的患者也明显较多,差异有统计学意义(P<0.05)。增强CT胃间质瘤总检出率为95.35%,超声双重造影胃间质瘤总检出率为86.05%,联合检测胃间质瘤检出率高达97.67%,高于两种方法单独检测,但差异不具有统计学意义(P>0.05)。增强CT定位准确率为65.85%,超声双重造影定位准确率为97.30%,联合检测定位准确率高达100%,较单独检测差异显著(P<0.05)。结论:增强CT与超声双重造影在不同类型胃间质瘤的定位及定性诊断中各具优势,二者联合使用可显著提高胃间质瘤诊断准确率,值得临床推广。
Objective: To investigate the clinical value of gastric enhanced CT and ultrasound in the diagnosis of gastric stromal tumors. Methods: Forty-three patients with gastric stromal tumor who were treated in our hospital from January 2010 to January 2016 were retrospectively analyzed. Results of two contrast-enhanced ultrasonography and gastric enhanced CT were retrospectively analyzed. Diagnostic accuracy. Results: 43 cases of gastric stromal tumors were divided into high-risk type in 17 cases and low-risk type in 26 cases, which were divided into high-risk group and low-risk group. The number of high-risk patients with tumor size> 5 cm was significantly higher than that of low-risk patients, and the patients with liquefaction and necrosis were also significantly more, the difference was statistically significant (P <0.05). The overall detection rate of enhanced gastric stromal tumor was 95.35%, the total detection rate of ultrasound double-contrast gastric stromal tumor was 86.05%, and the detection rate of combined detection of gastric stromal tumor was as high as 97.67%, higher than the two methods alone, But the difference was not statistically significant (P> 0.05). The accuracy of enhanced CT localization was 65.85%, that of ultrasound double contrast imaging was 97.30%, and that of joint detection and localization was up to 100%, which was significantly different from the single detection (P <0.05). Conclusion: Contrast enhanced dual - contrast enhanced ultrasound and computed tomography have their own advantages in the localization and qualitative diagnosis of different types of gastric stromal tumors. The combination of them can significantly improve the diagnostic accuracy of gastric stromal tumors and is worthy of clinical promotion.
