目的了解依地酸钙钠(EDTA)对职业性慢性铅中毒的肾功能影响,为临床安全用药提供参考。方法回顾分析本院收治的73例职业性慢性铅中毒患者临床资料,对驱铅治疗前后血铅,肾小球功能中的尿素氮(BUN)、肌酐(CR)、尿酸(UA)和近端肾小管功能中的尿β2微球蛋白(β2-MG)和尿视黄醇结合蛋白(RBP)等结果进行综合分析。结果驱铅治疗可使患者近端肾小管功能中尿RBP明显好转(P<0.05),肾小球功能中UA升高(P<0.05)。结论临床常规驱铅治疗安全有效,但不要随意大剂量和长疗程使用,同时做好驱铅治疗期间UA的监测。
Objective To understand the effect of EDTA on the renal function of occupational chronic lead poisoning, and to provide a reference for clinical safety medication. Methods The clinical data of 73 patients with occupational chronic lead poisoning admitted to our hospital were retrospectively analyzed. The contents of BUN, CR, UA, Urine β2 microglobulin (β2-MG) and urinary retinol binding protein (RBP) in the renal tubular function were analyzed comprehensively. Results Lead extubation could significantly improve urine RBP (P <0.05) and glomerular UA in patients with proximal tubular function (P <0.05). Conclusion The conventional routine lead-removing therapy is safe and effective. However, it should not be used in large doses and long-term courses, and the monitoring of UA during the lead-removing treatment should be done at the same time.
